The debate between arthroscopic Bankart repair versus the Latarjet technique for patients with anterior shoulder instability largely hinges on the argument between redislocation rate versus complication rate. This difference becomes more apparent when considering patients with bone loss. We believe that Bankart "plus" techniques, including arthroscopic anatomic bone grafting, create a stable shoulder with a low instability recurrence rate, while maintaining a complication rate comparable to the Bankart repair.

Purpose: To compare functional outcomes, recurrence rate, range of motion (ROM) and return to sport between arthroscopic Bankart repair with remplissage (BR) and open Bankart repair with inferior capsular shift (OBICS) in contact and collision athletes with recurrent anterior shoulder instability.
Methods: A prospective comparative cohort study of 90 patients separated into two study groups (OBICS and BR) of 45 collision and contact athletes each was conducted. All athletes had subcritical glenoid bone loss (SGBL) ≤ 10% and off-track Hill Sach lesions (HSLs).

Introduction: Open Bankart repair plus inferior capsular shift has not yet been tested under the concept of glenoid track as a predictor of failure. The aim of this study was to compare the subjective and objective outcomes in collision athletes with subcritical glenoid bone loss and on-track Hill Sachs lesions versus those with off-track Hill Sachs lesions, all treated with open Bankart repair.
Methods: Two study groups were created: 50 patients had on-track Hill Sachs lesions, while 38 had off-track lesions.
Arthroscopic remplissage has been suggested as a means to decrease recurrent instability in arthroscopic Bankart repair surgery involving patients with significant Hill-Sachs lesions. Remplissage fills the Hill-Sachs lesion to prevent glenohumeral "engagement" and recurrent instability. Remplissage is not generally indicated in patients with smaller, nonengaging Hill-Sachs lesions.
