Complete surgical resection of intrahepatic cholangiocellular carcinoma is considered the primary option for curative treatment. With tumour extension, resection and reconstruction of vascular structures can be necessary. Three dimensional planning of surgical resection can be useful in selected cases. The current video shows an extended right sided hemihepatectomy with hilar resection and reconstruction of the left portal vein and one of two branches of the left hepatic vein in a patient with intrahepatic cholangiocarcinoma.

There are a variety of surgical conditions impacting the canine vagina and vestibule that require access through a caudal approach. A standard vertical episiotomy involves making an incision beginning ventral to the anus and extending ventrally through the dorsal commissure of the vulva and into the lumen of the vestibule and distal vagina. The objective of this study was to determine if complex vaginal procedures could be performed via a transverse perineal approach, thus eliminating the need to incise and reconstruct the vulva as performed in a standard episiotomy, and to evaluate the feasibility of vaginal resection with vulvar-sparing vestibular urethrostomy using this transverse approach.

Background: The main technique for identification of free flap perforator vessels is Doppler sonography, which is not always accurate, user dependent and affected by the patient's body habitus.
Methods: Adult patients undergoing head and neck resection and free flap reconstruction at two academic institutions were enrolled. Doppler sonography was used to identify perforators, and were marked using a skin marker.

Purpose: The medial collateral ligament (MCL), and posterior oblique ligament (POL) are the primary valgus stabilisers of the knee, and clinical examinations in grading valgus instability can be inherently subjective. Stress radiography of medial-sided knee injuries provides objective diagnosis and was analysed in this study. We hypothesised that (1) medial joint space opening would increase cutting the superficial MCL (sMCL), POL and anterior cruciate ligament (ACL); (2) isolated deep MCL (dMCL) injury would not increase medial joint space opening; (3) medial joint space opening would increase at higher flexion angles.
