Introduction: Intussusception is a relatively common condition seen in children. In comparison, adult intussusception is rare and is often secondary to inflammatory diseases, benign or malignant tumors and motility disorders. Being a benign cause, lipomas appear as a particularly rare gastrointestinal tumor.

Presentation Of Case: We present a case of colo-colonic intussusception secondary to a sigmoidal lipoma, in a 40-year-old man. We describe the different aspects of diagnosis and management of this rare complication.

Discussion: Adult intussusception is the cause of symptomatic bowel obstruction in 1% of cases and its colo-colonic occurrence represents 17% of all intestinal intussusceptions. The case that we describe is particularly unique because apart from being an example of intussusception in adults, it occurred in the sigmoid colon and was not associated with a malignant lesion. The treatment of intussusception in adults is surgical resection because of the high incidence of underlying malignancy. Colonoscopy is a modality which allows direct visualization of the lipoma. However, intraluminal reduction via colonoscopy is not recommended.

Conclusion: Colo-colonic intussusception is a very rare complication of lipoma. It is determined that the treatment is surgical due to the risk of malignancy.

Colonic lipomas are rare benign tumors that may appear throughout the entirety of the gastrointestinal tract, with a predisposition to appear in the colon. Patients with colonic lipoma are typically asymptomatic, making their diagnosis rare and incidental. This case report intends to investigate and clarify the decision-making process regarding surgical segmental colonic resection versus local excision via the study of a 48-year-old man with ulcerative colitis diagnosed in 2006, undergoing treatment with infliximab and without exacerbations since 2010.

Article Synopsis
  • * Sometimes, these lipomas can lead to issues like bleeding or blockages, but they are very rare, especially in adults.
  • * Doctors can use special scans to find CLs, but they need to be careful because they look similar to more serious tumors, sometimes requiring surgery to be sure.

Large colonic lipomas exceeding 2.0 cm in size may rarely mimic colorectal adenomas and hence we referred to such lipomas as pseudo-pedunculated lipomas. They can exhibit surface erosions, fissure ulcerations, and mucosal erythema and can pose diagnostic dilemmas during colonoscopy.

Key Clinical Message: Adult intussusception commonly has a leading point. In the colon, malignancy is a prevalent etiology for the leading point; however, benign tumors should also be considered. We present a case of colorectal intussusception caused by a giant lipoma.

Linear endoscopic ultrasonography (EUS) has been extensively utilized as a novel diagnostic and therapeutic modality across various fields. However, there have been relatively few studies focusing on lower gastrointestinal lesions. The aim of our study was to investigate the feasibility, safety and clinical value of linear EUS in the lower gastrointestinal subepithelial lesions.
