Rationale: We report a 45-year-old male patient with unilateral brachial plexopathy following acute carbon monoxide (CO) intoxication.
Patient Concerns: The patient suddenly developed severe left upper extremity weakness and cognitive impairment after CO intoxication.
Diagnoses: He showed severe weakness in the left upper extremity and decreased cognitive function during physical examination. Whole body bone scan revealed increased radioisotope uptake in the left shoulder, suggesting rhadomyolysis. Brain magnetic resonance imaging showed high signal intensity in both globus pallidus. The electrodiagnostic finding was compatible with incomplete left total brachial plexopathy (axonopathy).
Interventions: High oxygen therapy and massive intravenous normal saline infusion were administered immediately after acute CO intoxication. Since then, intensive rehabilitation treatment has been provided.
Outcome: Despite having received medical and rehabilitation treatment, the patient has not recovered severe weakness in the left upper extremity and decline in cognition.
Lessons: Localized swelling in the left shoulder caused by rhabdomyolysis may be a key mechanism in developing unilateral brachial plexopathy after acute CO intoxication. The early diagnosis and treatment of rhadomyolysis might be important preventing peripheral neuropathy. An electrodiagnostic study may be helpful for diagnosis of peripheral neuropathy after CO intoxication and prediction of patient's prognosis.

Purpose: The purpose of this case report is to present an anatomical variation of the radial artery observed in anatomical practice and to discuss its clinical significance.
Methods: During the dissection of the right upper limb of a Chinese adult male, a high-position variation of the right radial artery with the absence of the radial recurrent artery was found.
Results: The variant branch of the right radial artery originates from the brachial artery and arises medially in the upper one-third of the arm.

Objective: To analyze the clinical value of the combination of esketamine and ropivacaine in alleviating rebound pain in patients with upper limb fractures following brachial plexus block.
Methods: A total of 149 patients with unilateral upper limb fractures who underwent open reduction and internal fixation surgery under brachial plexus block anesthesia from November 2021 to August 2022 were selected as the subjects for the study and randomly divided into the esketamine group (RNK group) and the ropivacaine group (R group). The incidence of rebound pain at 48 h postoperatively, intraoperative mean arterial pressure (MAP) and heart rate (HR), the onset time and duration of blockade, the Numeric Rating Scale (NRS) scores for pain at rest and with activity during the perioperative period, the dosage, numbers of compressions, and effective compressions of postoperative patient-controlled analgesia with sufentanil, and occurrence of adverse reactions were assessed and compared between the two groups.
