AI Article Synopsis

  • The study aimed to evaluate the effectiveness of high frequency ultrasound in measuring skin thickness in patients with systemic sclerosis (SSc) compared to healthy individuals.
  • Data was collected from 82 SSc patients and 67 healthy volunteers, where skin thickness was measured at various body locations and correlated with the modified Rodnan skin score (mRSS).
  • Results showed significant increases in skin thickness in SSc patients, particularly in the fingers and forearms, with high diagnostic accuracy for distinguishing SSc from healthy skin based on specific cut-off values of skin thickness.

Article Abstract

Objective: To investigate the performance of high frequency ultrasound in the assessment of skin thickness in patients with systemic sclerosis (SSc).

Methods: The study included 82 SSc (SSc group) and 67 healthy volunteers (control group) from 2014 to 2016. The skin thickness at bilateral middle fingers and forearms, anterior chest and abdominal wall was measured using high frequency ultrasound. All the patients with SSc underwent the modified rodnan skin score (mRSS) over 17 anatomical sites by an experienced dermatologist. The differences in age, sex, height, body mass, body mass index (BMI) and skin thickness between SSc patients and healthy controls were compared. Receiver operating characteristic (ROC) curve analysis was performed to determine the performance of high frequency ultrasound in the differentiation of SSc from healthy skin, and the correlation of mRSS with skin thickness were analyzed.

Results: SSc patients and healthy controls shared similar demographic features (age, sex ratio, height, body mass,BMI) (>0.05). Skin thickness values in SSc patients were increased significantly at fingers and forearms compared with healthy controls (<0.05). The area under the curve () was 0.938, 0.905, 0.608, 0.586, 0.398, 0.321 at right and left finger, right and left forearm, chest and abdominal wall. Among them,>0.9 of right and left fingers can be used for diagnosis,The skin thickness cut-off value for determining the diagnosis of SSc were as follows: 1.35 mm at the right finger with 84.1% sensitivity and 95.5% specificity,1.26 mm at the right forearm with 86.6% sensitivity and 89.6% specificity, respectively. Skin thickness increased significantly with mRSS. The correlation of total mRSS scores with total skin thickness was 0.599 (<0.001),and the correlation of local mRSS score with local skin thickness were 0.400-0.623 (<0.001),with the highest correlation coefficient at right finger and the lowest at abdomen.

Conclusion: High frequency ultrasound may reflect extent of skin involvement of SSc,and skin thickness assessed with high frequency ultrasound appeared to be highly specific and sensitive at fingers.

Vascular Basement Membrane Fragmentation in Keloids and the Expression of Key Basement Membrane Component Genes.

Background: Keloids are growing scars that arise from injury to the reticular dermis and subsequent chronic local inflammation. The latter may be promoted by vascular hyperpermeability, which permits the ingress of chronic inflammatory cells/factors. Cutaneous capillaries consist of endothelial cells that generate, and are anchored by, a vascular basement membrane (VBM).

Introduction: Silver sulfadiazine 1 % cream had historically been the mainstay initial treatment for scald wounds at our institution. However, we transitioned to using closed dressings of only petrolatum-impregnated 3 % bismuth tribromophenate gauze (Xeroform) for all partial-thickness burns. Xeroform adheres to the wound while allowing the exudates to drain and acts as a scaffold for re-epithelialization, after which it falls off without traumatizing the wound bed, theoretically allowing viable tissue to declare itself while requiring less frequent dressing changes.

Background: The skin barrier plays a crucial role in protecting our body against external agents. Disruption of this barrier's function leads to increased susceptibility to infections and dermatological diseases. Damaged skin can be due to the use of detergents, sunburn or excessive scratching.

Department of Rheumatology and Immunology, Xiangya Hospital, Central South University, Changsha, Hunan, 410008, P.R. China.

Background: ADAM19 (ADAM Metallopeptidase Domain 19) is known to be involved in extracellular matrix (ECM) remodeling, yet its specific function in systemic sclerosis (SSc) fibrosis remains unclear.

Objectives: This study sought to clarify the role and underlying mechanism of ADAM19 in SSc skin fibrosis.

Methods: The expression of ADAM19 was assessed in skin tissues of SSc and wound healing using publicly available transcriptome datasets.
