This case report presents an early clinical evaluation of a patient with bilateral cleft lip and palate (CLP) treated with ArchWise Distraction Osteogenesis Technique and Appliance. A 16-year-old patient with a bilateral CLP had a chief complaint of missing upper anterior teeth and poor aesthetics. The patient had class III skeletal and dental relationships, maxillary hypoplasia, maxillary posterior arch constriction, premaxillary deficiency, multiple missing teeth due to an alveolar cleft, and a concave profile. Treatment plan consisted of maxillary expansion with quad helix appliance, leveling and aligning of the arches with fixed orthodontic appliances, and archwise distraction osteogenesis. Osteotomies were performed to mobilize the posterior segments. Activation of the distractors was continued until the lateral segments had contacted each other, and then sagittal distraction was performed in order to correct the sagittal discrepancy. Patient was referred to the oral surgeon and prosthodontist for the final restorations. A very large gap was successfully repaired using this protocol while maintaining the ideal arch form and generating new bone behind the distracted segments.

Limb lengthening and deformity correction techniques, particularly distraction osteogenesis, have significantly evolved in pediatric orthopedics. This study examines the temporal changes of key biochemical markers-vascular endothelial growth factor (VEGF), fibroblast growth factor 1 (FGF-1), and the propeptide of type I collagen (P1NP)-during the limb lengthening process. Twenty pediatric patients (aged 13-16) underwent distraction osteogenesis using the Circular Hexapod External Fixator.

Objective: To describe the characteristics of short arm deformity in patients with achondroplasia, and summarize the progress of its lenthening and reconstruction, so as to provide reference for clinical diagnosis and treatment.

Methods: The literature on the lenthening of upper limb with achondroplastic short arm deformity at home and abroad in recent years was reviewed, and the characteristics, extension methods, postoperative management, effectiveness evaluation, and related complications of short arm deformity were summarized.

Results: Achondroplastic short arm deformity affect the patient's daily perineal hygiene activities.

Optimal age for mandibular distraction osteogenesis in craniofacial microsomia: A systematic review and meta-analysis.

J Stomatol Oral Maxillofac Surg

January 2025

Department of Maxillofacial Surgery, Plastic Surgery Hospital and Institute, Chinese Academy of Medical Sciences, Peking Union Medical College. No. 33, Ba-Da-Chu Rd, Shi Jing Shan District, Beijing 100144, PR China. Electronic address:

Objective: This systematic review and meta-analysis aimed to determine the optimal timing for mandibular distraction osteogenesis in children with craniofacial microsomia based on long-term outcomes and relapse rates.

Methods: A comprehensive search of PubMed, Embase, Cochrane Library, and Web of Science was conducted for studies published up to September 2024. Eligible studies included children aged ≤16 years with CFM who underwent MDO, with follow-up duration exceeding one year.
