Unique case of retained needle in the AirSeal trocar during robot-assisted laparoscopic radical prostatectomy. A 68-year-old male with prostate cancer underwent robot-assisted laparoscopic radical prostatectomy. Upon laparoscopic removal of final intra-abdominal suture by bedside assistant, needle became dislodged from suture and was unable to be located after a standard systematic search. Ultimately, needle was found caught in the assistant's AirSeal trocar device. Intraoperative loss of a foreign body should include inspection, and possible radiographic evaluation, of the trocar mechanism as part of a complete systematic approach.

Background: Valveless trocars like AirSeal system are maintain a stable pneumoperitoneum and reduce instrument friction.
Case Presentation: A 65-year-old man's robotic radical cystectomy was complicated by a missing needle while using AirSeal system. The needle was detected via backward inspection using the endo camera inserted through the trocar, tip at its most distal end let the visualization of the needle within the air channels and confirmed with a trocar X-ray.

Purpose: AirSeal is a valve-less trocar insufflation system which is widely used in robotic urologic surgeries. More evidence is needed concerning the application and cost of AirSeal in retroperitoneal robot-assisted laparoscopic partial nephrectomy.

Conventional operative insufflation uses a one-way trocar to handle instruments while maintaining pneumoperitoneum. In 2007, the AirSeal valveless trocar insufflation system was introduced, which maintains stable pneumoperitoneum while continuously evacuating smoke. Although this device has been validated in adult patients, it has not been extensively validated in the pediatric population.
