Medial column (deltoid ligament) integrity is of key importance when considering the stability of isolated lateral malleolus ankle fractures.Weight-bearing radiographs are the best method of evaluating stability of isolated distal fibula fractures.Computed tomography (CT) scanning is mandatory for the assessment of complex ankle fractures, especially those involving the posterior malleolus. Most isolated trans-syndesmotic fibular fractures (Weber-B, SER, AO 44-B) are stable and can safely be treated non-operatively.Posterior malleolus fractures, regardless of size, should be considered for surgical fixation to restore stability, reduce the need for syndesmosis fixation, and improve contact pressure distribution. Background: The Clinical Frailty Scale (CFS) is a tool to assess the overall health of older adults. There are few reports of CFS and prognosis of ankle fracture. The objective of this study was to determine the predictive power of the CFS for adverse clinical and radiographic outcomes after surgery in elderly patients with trimalleolar fractures.

Management of ankle fractures necessitating surgery is impacted by soft tissue swelling, often delaying open reduction and internal fixation (ORIF). Subjective evaluations are inconsistent, and traditional measurement methods are often inaccurate or impractical. This prospective observational study included 17 adults with unilateral closed ankle fractures requiring surgical fixation.

Case: A 30-year-old man presented with left proximal tibia fracture (open Grade 3A) and a transverse lateral malleolus fracture (Weber B) following direct impact injury. Computed tomography revealed a rare posteromedial tubercle fibula fracture at the posterior inferior tibiofibular ligament (PITFL) insertion, with syndesmosis disruption. Initial damage control included an external fixator.

Ankle fractures, often accompanied by other injuries and complications, pose a significant healthcare burden due to their high incidence and associated treatment challenges. This case report investigates the use of Cytal™ Wound Matrix, derived from urinary bladder matrix (UBM), in managing postoperative complications following open reduction and internal fixation (ORIF) of a trimalleolar ankle fracture. A 57-year-old male with a history of hypertension sustained a trimalleolar ankle fracture following a road traffic accident.

The deltoid ligament (medial collateral ligament) and the syndesmosis (a composite ligamentous structure at the distal tibiofibular junction) are critical for maintaining ankle stability. In cases of high-energy ankle fractures, these structures are often injured simultaneously, leading to instability and potential long-term complications such as post-traumatic arthritis. This review aims to explore advancements in minimally invasive techniques for the treatment of combined deltoid ligament and syndesmosis injuries, with a focus on optimizing surgical outcomes and reducing patient morbidity.
