The cytopathologic features of a malignant granular cell tumor in both exfoliative and aspiration cytopreparations included isolated cells with low nuclear-cytoplasmic ratios and abundant, diffusely granular cytoplasm. Transmission electron microscopy revealed the diagnostic secondary lysosomes in the neoplastic cells.

Biomedical scientists (BMS) can report sample adequacy during EBUS TBNA using rapid on-site evaluation (ROSE). Despite being able to report exfoliative samples such as pleural fluid cytology and bronchial washings, they are usually not permitted to offer a preliminary diagnostic impression of EBUS TBNA samples. Experienced biomedical scientists can provide a reliable diagnostic impression during ROSE for EBUS samples, with sensitivity and specificity comparable to cytopathologist reporting.

Personalized therapy in lung cancer (LC) has revolutionized routine histopathology and cytopathology, emphasizing the importance of obtaining adequate material for molecular studies to support oncological decisions. Adaptations of cytologic sample preparations offer benefits for molecular testing, yet their potential remains underutilized. A significant number of LC cases is identified through specimens of aspiration or exfoliative cytology.

Introduction: Ancillary testing on cytopathology and other small biopsy specimens is crucial for diagnosis and provides critical information to clinicians. Testing is dependent on preanalytic factors and would benefit from standardization of specimen collection protocols across laboratories. To assess institutional practices and areas of need for evidence-based standards, we surveyed current practices across cytopathology laboratories.
