Published: October 2018

The following review summarizes the pro-con debate about current controversies regarding the pathogenesis of pulmonary arterial hypertension (PAH) that took place at the American Thoracic Society Conference in May 2017. Leaders in the field of PAH research discussed the importance of the immune system, the role of hemodynamic stress and endothelial apoptosis, as well as bone morphogenetic protein receptor-2 signaling in PAH pathogenesis. Whereas this summary does not intend to resolve obvious conflicts in opinion, we hope that the presented arguments entice further discussions and draw a new generation of enthusiastic researchers into this vibrant field of science to bridge existing gaps for a better understanding and therapy of this fatal disease.

Off-Pump Coronary Artery Bypass Grafting is Overutilized.

Off-pump coronary artery bypass grafting (CABG), developed to avoid the potential complications of cardiopulmonary bypass, remains a subject of debate. Studies have demonstrated that off-pump CABG is associated with higher rates of incomplete revascularization, inferior graft patency, and increased reintervention rates compared to on-pump CABG, leading to worse outcomes. The theoretical neuroprotective and renal-protective benefits associated with off-pump CABG have not been definitively proven, with stroke and renal failure rates similar to those of on-pump CABG in both short- and long-term follow-up.

In this Pro-Con commentary article, we discuss whether anesthesiologists should be responsible for treating preoperative anemia. This debate was proposed based on the article published in this issue of Anesthesia & Analgesia by Warner et al, which is an advisory on managing preoperative anemia endorsed by both the Society of Cardiovascular Anesthesiologists and the Society for Advancement of Patient Blood Management. All evidence suggests that anemia is not just an innocent "bystander" which can be solved by transfusion.

To biopsy or not to biopsy a teenager with idiopathic nephrotic syndrome? Biopsy first.

Pediatr Nephrol

February 2025

Nephrology, Dialysis and Transplantation Unit, IRCCS Istituto Giannina Gaslini, Via Gaslini 5, 16147, Genoa, GE, Italy.

Article Synopsis
  • Kidney biopsy is important for figuring out kidney problems in kids, especially to see if treatments will work differently based on what the biopsy shows.
  • For kids over 12 with a certain condition called idiopathic nephrotic syndrome (INS), doctors might consider a biopsy because they could have different kidney issues than younger kids.
  • Some experts think biopsies should be done before starting treatment, like steroids, because the biopsy can help predict how well the treatment will work, especially for serious cases.
