Drug-induced lupus erythematosus (DILE) is an autoimmune reaction that results in symptoms of polyarthralgia, fever, and cutaneous lesions and other manifestations. Several drugs have been documented to cause this disease, including procainamide, isoniazid, methyldopa, penicillamine, and hydralazine. Systemic lupus erythematosus (SLE) manifestations often occur after the patient has been taking the drug without complications for months to years.

Antibiotics are indicated for inappropriately prolonged durations not only for infectious diseases but also for non-infectious dermatologic conditions such as acne vulgaris, but inappropriate use is not uncommon. Inappropriate antibiotic prescription can contribute to the emergence of multidrug-resistant organisms, patient harm such as adverse drug events and increased healthcare costs. We experienced a case involving a late adolescent with acne vulgaris who had been treated with minocycline for more than 2 years, leading to the development of drug-induced lupus, attributed to minocycline.

A 46-year-old man with a known case of Crohn's disease, which developed in August 2010, had been in remission since then with infliximab treatment. However, in November 2023, he developed photosensitivity, followed by joint pain and general fatigue in December. Blood tests revealed positive antinuclear antibodies, leading to a diagnosis of drug-induced lupus.
