Background: Identification and characterization of non-typeable Haemophilus influenzae (NTHi) with reduced susceptibility to β-lactam antibiotics due to mutations in penicillin binding protein 3 (PBP3) is a clinical challenge. We analyzed a blood isolate, NTHi93-57485, that was categorized as aminopenicillin resistant but lacked key amino acid substitutions in PBP3 that have previously been associated with reduced aminopenicillin susceptibility. The significance of an alternative amino acid substitution (Y528H) in this isolate was examined.

Results: Site-directed mutagenesis of a β-lactam susceptible H. influenzae (NTHi3655) was performed to introduce the Y528H substitution into wild-type ftsI (encoding for PBP3). Disc diffusion screening and broth microdilution determination of MICs for β-lactam agents were done with the NTHi3655-PBP3 mutant and were compared with the NTHi3655 wild-type as well as the original clinical isolate NTHi93-57485. Introduction of the Y528H substitution in NTHi3655 resulted in positive screening for β-lactam resistance. MICs for aminopenicillins were increased in the mutant compared to the wild-type. However, the mutant remained susceptible to aminopenicillins according to EUCAST clinical breakpoints (assuming intravenous treatment) and the introduction of Y528H alone did not increase the resistance to the same level as NTHi93-57485. None of the isolates had frame shift insertions in the acrR gene regulating the AcrAB efflux pump.

Conclusions: In parallel to the previously well-described PBP3-substitutions R517H and N526K, we demonstrate that Y528H confers reduced aminopenicillin susceptibility.

Pakistan has a large, intensive broiler production industry, where antimicrobials are extensively used for both therapeutic and prophylactic purposes. Monitoring antimicrobial use (AMU) at the farm level is crucial to guide interventions for antimicrobial stewardship. The objective of this study was to comprehensively quantify AMU on commercial broiler farms in Pakistan using different metrics.

Antibiotic prophylaxis in oral implant surgery in Germany: a cross-sectional study.

Int J Implant Dent

December 2024

Institute of Clinical Pharmacology, Immanuel Hospital Rüdersdorf, Brandenburg Medical School Theodor Fontane, Rüdersdorf, Germany.

Purpose: Prophylactic antibiotics are used in dental implants to reduce infection risk and implant failure, especially benefiting patients with risk factors. However, evidence suggests that using clindamycin or extending antibiotics postoperatively has an unfavorable risk-benefit ratio.

Methods: This national cross-sectional study analyzed antibiotic prophylaxis during implant insertion across Germany.

In recent years, several threatening developments regarding antibacterial drugs, such as rising bacterial resistance and delivery bottlenecks, have occurred. Since antibacterial drugs are crucial for modern medicine, understanding events and influencing factors relevant for long-term developments is essential. Therefore, we analyzed the number of prescriptions and costs, defined daily dose (DDD) and DDD costs of antibacterial drugs in Germany, based on the Arzneiverordnungsreport (AVR, Drug description report) between 1985 and 2022.

Article Synopsis
  • Antimicrobial resistance (AMR) is projected to cause 10 million deaths annually by 2050, with 1.27 million deaths directly resulting from antibiotic resistance (ABR) in 2019, heavily impacting children under five.
  • The study aims to compare the impact of ABR in terms of death and disability-adjusted life years (DALYs) between Italy and Western Europe, identifying urgent healthcare needs in Italy.
  • Data was extracted from the MICROBE tool, analyzing the burden of 33 bacterial pathogens across various infectious syndromes, and comparing death and DALY rates related to ABR in both regions to highlight significant health challenges.

Article Synopsis
  • The French National Authority for Health reviewed literature on complications from pelvic organ prolapse (POP) surgery using synthetic mesh, which are rare but can severely impact women's quality of life, leading to multidisciplinary clinical practice recommendations with graded levels of evidence.
  • Patients must be adequately informed about the risks of POP surgery, with specific guidelines suggesting against certain practices like vaginal packing and requiring alternative surgical techniques in complex cases.
  • For bladder injuries during surgery, recommended practices include suturing and monitoring ureter permeability, with potential use of a prosthetic mesh post-repair for better outcomes.
