Explantation of a failed endovascular stent graft in a patient with a type B dissection.

We report a patient who underwent insertion of an endovascular stent graft in the descending aorta for an aneurysmal type B dissection. The patient developed a proximal type I endoleak which required explantation of the graft and replacement of the descending aorta. Carotid artery cannulation was utilized for antegrade perfusion during the period of circulatory arrest.

Hydrophilic coating embolism (HCE) is a rare and underreported complication in neurointerventional practice that can lead to serious medical consequences. Two endovascular procedures were interrupted at our institution after a cloudy liquid content was observed inside the rotating hemostatic valves (RHV) during microcatheter withdrawal. In both cases, the same type of microcatheter (Prowler Select Plus) and RHV (Merit) were being used, and coating dislodgement was suspected.

Objective: The aim of this study was to perform a comparative pre-clinical evaluation of a new fenestrated endovascular aneurysm repair dedicated stent using perfused 3D printed patient anatomies.

Methods: The test setup included a pulsatile pump set to reproduce human haemodynamics, four 3D printed pararenal aneurysms connected to a bench test, and four corresponding fenestrated grafts (Zenith Fenestrated; Cook, Bloomington, IN, USA). Bridging stents were sized based on analysis of patients computed tomography scans and included either four standard covered stents (BeGraft [BG]; Bentley, Hechingen, Germany) or four similar covered stent grafts mounted on a modified balloon to allow stent implantation and flaring in one step (BeFlared [BF]; Bentley).
