Unlabelled: We describe the treatment of 101 patients with 113 fractures of the proximal phalanx at the junction of the diaphysis with the proximal metaphysis. Fractures were stabilized using a single extra-articular percutaneous Kirschner wire passed lateral to the metacarpal head into the base of the phalanx base and across the fracture to engage the lateral cortex. Patients were treated semi-electively on a day surgery unit. The metacarpophalangeal joint was splinted in flexion with mobilization of the proximal inter-phalangeal joints, until the wire was removed at 24 days and the hand then freely mobilized. The mean total active movement was 230° and no patients experienced problems with significant pain. There were two pin site infections. There were no cases of angular or rotational malunion. These injuries can be successfully and safely treated using this simple technique, negating the need for close monitoring for angular displacement of the fracture, which is frequently seen with non-surgical management.

Level Of Evidence: IV.

Anatomical and Surgical Neck Circumference of Adult Human Humerus.

Mymensingh Med J

January 2025

Dr Shah Md Atiqul Haque, Assistant Professor, Department of Anatomy, Mymensingh Medical College (MMC), Mymensingh, Bangladesh; E-mail:

The upper end of the humerus consists of three necks such as anatomical neck, surgical neck and morphological neck. The anatomical neck of the humerus is the area just below the head of the humerus. The surgical neck of the humerus is the area just below the greater and lesser tubercle of the humerus.

Background: Distal radius fractures are prevalent in small and toy-breed dogs, presenting significant treatment challenges due to complications such as delayed union or non-union. These complications are often exacerbated by reduced vascular density at the distal diaphyseal-metaphyseal junction of the radius, which is vital for bone healing, particularly in toy and small breed dogs. Circular external fixation (CEF) is known for its effectiveness in managing acute and chronic fractures and providing temporary stabilization in humans.

Background: To compare the efficacy of intramedullary nailing via the lateral parapatellar approach versus the infrapatellar approach in treating fractures at the tibial metaphyseal-diaphyseal junction.

Methods: A retrospective analysis was conducted on the clinical data of 45 patients with proximal or distal tibial fractures treated with intramedullary nailing via lateral parapatellar approach (n = 23) or infrapatellar approach (n = 22) between January 2019 and March 2023. We recorded and compared the operative time, intraoperative blood loss/fluoroscopies, success rate of closed reduction, anteroposterior and lateral entry point accuracy, postoperative infection, fracture healing time, as well as NRS pain scores, Lysholm knee function scores, and knee range of motion.

Introduction: Resistant and refractory non-union of distal tibia extra-articular fractures is very common even following internal fixation due to poor blood supply to the distal tibia metaphyseal region. The management can be challenging.

Case Report: A 50-year-old diabetic male had sustained closed distal tibia extra-articular fracture at the junction of diaphysis and metaphysis.

Free non-vascularised fibular grafts are an autologous grafting option that requires a simple technique and minimal expertise to replicate. However, due to limited intrinsic biological activity, they are less preferred than their vascularised counterparts for reconstructing large bone defects. We present a case of a female child in her middle childhood with limb length discrepancy due to extensive femoral diaphyseal bone loss following successful treatment of chronic osteomyelitis of the femur.
