A 20-year-old female patient was admitted to hospital because of bilateral leg weakness. Laboratory investigation showed metabolic alkalosis and severe hypokalemia. Differential diagnosis included mineralocorticoid or apparent mineralocorticoid excess diseases, with a high aldosterone-to-renin ratio (ARR) after correcting hypokalemia. After confirmatory tests, imaging studies revealed a unilateral adrenocortical adenoma consistent with Conn's disease. Surgery was curative.

Hereditary spastic paraplegia (HSP) is a rare neurodegenerative disease caused by retrograde degeneration of the corticospinal tract and posterior columns, which presents with progressive bilateral leg weakness and spasticity. HSP is inherited in an autosomal dominant pattern involving over 80 causative genes. The recently identified causative gene is the ubiquitin-associated protein 1 ()gene, which is associated with juvenile-onset pure spastic paraplegia-80 (SPG80).

Intradural extramedullary spinal cord tumors are rare but can cause significant neurological symptoms. We present a case of a 23-year-old male who developed progressive bilateral leg pain and lower limb weakness 2 years after undergoing posterior spinal fixation for a T12 burst fracture. Magnetic resonance imaging (MRI) revealed an intradural extramedullary tumor at the site of the previous surgery.
