The aim of this study was to assess the inflammatory cytokine response and possible association with antimicrobial treatment with penicillin, ceftriaxone, and doxycycline in acute leptospirosis. In the early acute stage, interleukin-10 (IL-10) levels were higher in mild cases than in severe cases ( = 0.01). IL-6 and IL-8 levels were low in patients who received >5 antimicrobial doses ( < 0.01). IL-8 levels were negatively correlated with the number of ceftriaxone doses administered ( = -0.315; = 0.031). Further studies are needed to evaluate the possible downregulation of proinflammatory cytokines by ceftriaxone in leptospirosis.

A protein subunit vaccine comprising conserved surface-exposed outer membrane proteins (SE-OMPs) is considered a promising platform for leptospirosis vaccine. The search for novel vaccine candidates that confer high protective efficacy against leptospirosis is ongoing. The LIP3228 protein was previously identified as a conserved and abundant SE-OMP with the potential to serve as an effective vaccine candidate.

Introduction Leptospirosis is a zoonotic infection caused by bacteria, which is reemerging in various regions and often poses a diagnostic challenge due to its nonspecific symptoms. While most infections are mild, severe cases occur in 5-10% of patients and are associated with high mortality, especially in areas with poor sanitation and urbanization. We present a rare and challenging case of Weil's disease in a patient who was admitted to the Critical Care Unit with a clinical suspicion of the condition. The patient showed a prompt response to treatment before laboratory confirmation was obtained. Leptospirosis, a zoonotic disease, is most commonly found in tropical and temperate regions.
