Minimising harms of tight glycaemic control in older patients with type 2 diabetes.

Afr J Prim Health Care Fam Med

December 2024

Department of Anaesthesiology, Pharmacology and Therapeutics, Faculty of Medicine, University of British Columbia, Vancouver.

In older adults with type 2 diabetes (T2DM), tight glycaemic control (HbA1c 7%) can result in more harm than benefit, especially when using insulin or sulfonylureas. Older adults are at higher risk for adverse drug events, especially hypoglycaemia, which may cause falls, confusion and hospitalisations. This Therapeutic Letter evaluates the risks of tight glycaemic control in older adults with T2DM, focusing on deprescribing diabetes medications in those over 65, especially those with multimorbidity and polypharmacy.

Introduction: Chronic inflammation caused by infections has a significant negative impact on the reproductive system and impairs fertility. The corpus luteum (CL) plays a central role not only in regulating the ovary cycle, but also in implantation of the embryo and maintenance of early pregnancy through the secretion of progesterone. Understanding the intricate interplay between inflammatory processes and reproductive organ's function is crucial for the development of effective therapeutic strategies to alleviate reproductive disorders and improve fertility.

Background: Metabolic dysfunction-associated steatotic liver disease (MASLD) has become one of the most prevalent chronic liver diseases worldwide. The serum uric acid-to-high-density lipoprotein cholesterol ratio (UHR) has been recognized as a novel marker for metabolic diseases, including MASLD. However, all previous studies were performed in adults.

Background: Type 2 diabetes mellitus (T2DM) is a metabolic disorder characterized by chronic hyperglycemia, mostly resulting from impaired insulin production and diminished glucose metabolism regulation. Qiwei Baizhu San (QWBZS) is a classic formula used in traditional Chinese medicine for the treatment of T2DM. A comprehensive analysis of the efficacy and safety of QWBZS in the treatment of T2DM is essential.
