When to deliver the monochorionic diamniotic (MCDA) twins with specific cord patterns? Although there is no clear evidence supporting an earlier delivery (before 36 weeks of gestation) in MCDA twins, an earlier delivery might prevent intrauterine death or neuromorbidity in MCDA twins with specific cord patterns.

Objective: Monochorionic diamniotic (MCDA) twins with amniotic fluid abnormalities that do not meet criteria for twin-twin transfusion syndrome (TTTS) concern physicians and families. This study aimed to describe the natural history of amniotic fluid abnormalities.

Methods: In this retrospective case-control study, TTTS screening ultrasounds and clinical records throughout all MCDA twin gestations were reviewed between 2018 and 2022 at a tertiary fetal care center.

Zinc is an essential trace element. The regulatory mechanism of zinc and its transporters in fetal growth in monochorionic diamniotic (MCDA) twins with selective intrauterine growth restriction (MCDA-sIUGR) is unclear. A total of 45 MCDA twins were divided into two groups, MCDA (n=37) and MCDA-sIUGR (n=8), to investigate their possible effects on fetal growth.

Twin pregnancies are at increased risk of morbidity and mortality compared to singletons. Among all twins, monochorionic pregnancies are at higher risk of specific and non-specific complications compared to dichorionic pregnancies. Therefore, it is of great importance to properly counsel future parents with monochorionic pregnancies regarding the risks of adverse outcomes and the modalities of monitoring and intervention of the potential complications.

Article Synopsis
  • - Twin pregnancies have a higher risk of complications like fetal growth restriction and twin-to-twin transfusion syndrome (TTTS), which can affect the fetal heart's hemodynamics.
  • - This paper reviews existing literature on fetal cardiac function in twin pregnancies using two-dimensional speckle tracking echocardiography (2D STE), finding significant differences in heart strain between donor and recipient twins.
  • - Although 2D STE proved feasible for studying monochorionic diamniotic (MCDA) twins, inconsistent results due to varying methodologies indicate a need for further research on its use in uncomplicated twin pregnancies.

Article Synopsis
  • This study focuses on the importance of monitoring fetal growth in twin pregnancies in China, where no clinically validated tools currently exist.
  • Researchers developed chorionicity-specific growth charts for ultrasound estimated fetal weight (EFW) and validated them using a study from a tertiary hospital involving twin pregnancies delivered between 2007 and 2021.
  • The results demonstrated significant differences in EFW between monochorionic and dichorionic twins, highlighting the need for tailored growth charts, and provided insights into neonatal outcomes based on different EFW classifications.
