[Frecuency of antigens and alloantibodies of Diego system in blood].

Introduction: The Diego blood group is an irregular blood system which has been involved in cases of hemolytic disease of the newborn and post transfusion reactions, in this system have been identified 22 erythrocyte antigens, among which the pair Di/Di is the most important because those have the most immunogenic potential.

Objective: This research aims to determine the frequency of antigen Dia and their respective alloantibody in the Ecuadorian population. Methods: It was performed a simple random sampling in the donor population, being later tested tube agglutination by the presence or absence of the antigen and its alloantibody Di applying gel agglutination technique.

Results: It was observed an antigen prevalence of 25% against a 6.09% of percentage alloimmunization due to Di antigen, without significant differences between men and women and these being independent of the age and origin of the donor, showing that there are some Diego positive cases in Ecuadorian population as probably cases of transfusional alloinmunization or due to fetal-maternal alloinmunization.

Conclusions And Recommendations: The frequency distribution of antigens and alloantibodies from the Diego blood group is almost uniform in the population, due presumably to the high incidence of miscegenation in our country. Therefore it becomes vitally important the implementation of this blood system inside the protocols of irregular antibodies identification in Ecuadorian blood banks.

Background:  We previously identified a factor (F)VIII molecular defect associated with an R2159C mutation in the C1 domain (named "FVIII-Ise") together with undetectable FVIII antigen (FVIII:Ag) levels measured by two-site sandwich ELISA using an anti-C2 domain alloantibody (alloAb). The patient had clinically mild hemophilia A, and his reduced FVIII:C correlated with FVIII:Ag measured by ELISA using monoclonal antibodies (mAbs) with A2 and A2/B domain epitopes, suggesting that the R2159C mutation modified C2 domain antigenicity.

The Clinical Significance of HLA Compatibility Scores in Lung Transplantation.

Lung transplantation is a life-saving therapeutic option for many chronic end-stage pulmonary diseases, but long-term survival may be limited by rejection of the transplanted organ. Since HLA disparity between donor and recipient plays a major role in rejection, we performed a single center, retrospective observational cohort analysis in our lung transplant cohort (n = 128) in which we calculated HLA compatibility scores for B-cell epitopes (HLAMatchmaker, HLA-EMMA), T-cell epitopes (PIRCHE-II) and missing self-induced NK cell activation (KIR Ligand Calculator). Adjusted Cox proportional hazards model was used to investigate the association between mismatched scores and time to development of donor-specific antibodies (DSA) post-transplant, time to first biopsy-proven acute rejection episode, freedom from CLAD, graft survival and overall survival.

Imlifidase is an endopeptidase known for cleaving anti-Human Leucocyte Antigen donor-specific antibodies (DSA) to allow high-risk kidney transplantation. However, it lacks comprehensive data regarding its effect on alloantibodies targeting other histocompatibility antigens, such as Major Histocompatibility Complex class I chain-related protein A (MICA). This study describes the dynamics of anti-MICA antibodies following imlifidase administration in a kidney transplant recipient with anti-MICA*002 preformed DSA.
