A patient undergoing percutaneous transluminal angioplasty of a critical proximal stenosis of his anterior descending coronary artery died as a result of coronary air embolism from a defective dilatation catheter. The probable mechanism responsible for this lethal complication is discussed. Simple precautions are described that will prevent its future occurrence.

: A 65-year-old male veteran presented to the Veterans Affairs Boston Healthcare System (VABHS) emergency department with progressive fatigue, dyspnea on exertion, lightheadedness, and falls over the last month. New bilateral lower extremity numbness up to his knees developed in the week prior to admission and prompted him to seek care. Common cardiovascular surgeries include coronary artery bypass grafting, cardiac valve replacement, radiofrequency ablation, and cardiac intervention surgery. Multiple postoperative complications, such as hypoxic encephalopathy, air embolism, retained intracardiac air, cognitive dysfunction and major adverse cardiovascular events, including heart failure, ischemic stroke, and myocardial infarction, may occur after these cardiovascular surgeries.
