A 42-year-old man had an unstable Jefferson type IV atlas fracture with unilateral vertebral artery occlusion after a diving accident. We performed C1-ring reconstruction with a crosslink rod and C2 fixation to directly reduce the fracture fissure. Within 6 h, cerebellar hemisphere infarction developed. After decompressive craniectomy, duroplasty, and release of the vertebral artery occlusion caused by the transfixing rod, a postoperative computed tomography angiogram showed that blood flow in the right vertebral artery improved. We suggest cautiously inserting screws into the fractured C1 lateral mass and gently tightening the crosslink rod to prevent distal migration of a thrombus.

Background: The presence of significant tortuosity in access routes to aneurysms can interfere with catheter guidance and manipulation and significantly impact treatment strategies.
Observations: In this report, the authors combined intentional staged aneurysm embolization with the construction of a new direct access route, which they call a "highway bypass," for a symptomatic posterior circulation cerebral aneurysm that was difficult to access with a catheter. Notably, the highway bypass is used for catheter passage, and technical tips should be considered.

Background: Intravascular injection of liquid adhesive hemostats is a rare but serious complication that can result in cerebral thromboembolism.
Observations: A 64-year-old female underwent orbitozygomatic craniotomy for posterior communicating artery aneurysm clipping with the routine use of a flowable hemostatic agent during extradural dissection. After placement of the aneurysm clip, flow was confirmed through the parent vessel and nearby branches.

Objective: Since the emergence of the Pipeline Embolization Device (PED), its off-label use has significantly improved the treatment effectiveness of complex intracranial aneurysms, including the treatment of recurrent aneurysms. Although PED is effective in this situation, there is still a lack of evidence-based medicine for its role in recurrent aneurysms after stent-assisted coiling. The aim of this study is to evaluate the safety and efficacy of PED treatment for recurrent aneurysms after stent-assisted coiling.

The article considers the issues of clinical presentation, diagnosis and treatment of arterial dissection, a common cause of ischemic stroke at a young age. The causes, predisposing factors and pathophysiological foundations of the development of this disease are discussed. It is noted that arterial dissection is often accompanied by non-specific clinical manifestations, which complicates the diagnosis.

Objectives: This study evaluated an automated deep learning method for detecting calcifications in the extracranial and intracranial carotid arteries and vertebral arteries in cone beam computed tomography (CBCT) scans. Additionally, a model utilizing CBCT-derived radiomics imaging biomarkers was evaluated to predict the cardiovascular diseases (CVD) of stroke and heart attack.
Methods: Models were trained using the nn-UNet architecture to identify three locations of arterial calcifications: extracranial carotid calcification (ECC), intracranial carotid calcification (ICC), and vertebral artery calcification (VAC).
