Unlabelled: Introduction Recent changes in surgical education have had an impact on our congenital training programmes. The mandate of the 8-hour workweek, a rapidly expanding knowledge base, and a host of other mandates has had an impact on the readiness of the fellows who are entering congenital programmes. To understand these issues completely, we interviewed the top congenital experts in the United States of America. The purpose of this paper is to share their insight and offer suggestions to address these challenges.

Methods: We used a qualitative thematic analysis approach and performed phone interviews with the top five congenital experts in the United States of America.

Results: Experts unanimously felt that duty-hour restrictions have negatively affected congenital training programmes in the following ways: current fellows do not seem as conditioned as fellows in the past, patient handoffs are not consistent with excellent performance, the mentor-mentee relationship has been affected by duty-hour restrictions, and fellows may be less prepared for real-world practice. Three positive themes emerged in response to duty-hour restrictions: fellows appear to be doing less menial task work, fellows are now better rested for learning, and we are attracting more individuals into the speciality. Experts agreed that congenital fellowships should be increased to 2 years. There was support for both the traditional and integrated residency pathways. Discussion We are in a new era of education and must work together to overcome the challenges that have arisen in recent years.

Urology Resident Autonomy in the Veterans Affairs Healthcare System.

J Surg Educ

February 2025

Department of Surgery, Veterans Affairs of New Jersey Healthcare System, East Orange, New Jersey. Electronic address:

Purpose: Surgical resident autonomy in procedures has been eroding over time, due to multiple factors that include duty hour restrictions, focus on operating time, complication rate, and trust among supervising physicians. This study examines whether urology residents at the Veterans Affairs hospitals (VA) have experienced decreased surgical autonomy and contributing factors.

Methods: The national VA Surgical Quality Improvement Program (VASQIP) was queried for the most common urologic procedures between 2004 to 2019 with resident involvement.

Laypeople's Perspective on Physician Work-Hour Restrictions in Japan: A Cross-Sectional Study.

J Patient Exp

Center for General Medicine Education, School of Medicine, Keio University, Shinjuku-ku, Tokyo, Japan.

The aim of the study was to examine laypeople's perspectives on the impending implementation of physician work-hour restrictions in Japan, which had received limited research attention. We conducted a nationwide cross-sectional study in January 2024. The participants were monitors of an internet survey company who responded to closed questions regarding the expected effect of work-hour regulations, along with an open-ended question regarding their expectations or concerns about these restrictions.

Introduction: Given the high incidence rate of breast cancer and shortage of fellowship trained specialists, general surgeons are frequently responsible for these patients. Residents have less operative exposure to breast surgery due to duty hour restrictions and decreased resident autonomy. We created a curriculum using human donors designed to teach junior residents to perform breast lumpectomy and sentinel lymph node biopsies.

Article Synopsis
  • Burnout and overwork are common issues for junior training physicians, leading to protests for better working conditions and a concerning number of professionals leaving the field.
  • Despite the implementation of duty hour restrictions in 2003 that limit residents to 80 hours a week, these measures have not effectively reduced burnout or improved working conditions over the last two decades.
  • The article argues for a dual approach that combines a meaningful reduction in working hours with improvements in the quality of both clinical and non-clinical work environments to better address resident burnout and enhance training outcomes.
