One duct, two orifices, no names! the anatomy of the biliary cystic duct.

The biliary cystic duct (CD) connects the gallbladder to the extra-hepatic bile duct, and the point at which it does this delineates the division between the common hepatic duct and the common bile duct. Its clinical relevance in disease, and importance during interventions relating to the gallbladder mean that its normal and variant anatomy has been described extensively in literature. However, an aspect not yet fully described includes naming of its two orifices on either end. This is highly relevant for surgical, endoscopic, and percutaneous procedures. We describe these as encountered in normal CD and biliary tree anatomy. The cholangioblastic variant of intrahepatic cholangiocarcinoma is a distinctive neoplasm that typically affects young women without underlying liver disease. Morphologically, it demonstrates solid, trabecular, and tubulocystic architecture, biphasic small cell-large cell cytology, and immunoreactivity for inhibin, neuroendocrine markers, and biliary but not hepatocellular markers. In 2021, our group identified a characteristic NIPBL::NACC1 gene fusion in cholangioblastic cholangiocarcinoma, and since then ~20 genetically confirmed cases have been reported in the literature.

Gall bladder duplication: A rare biliary malformation.

Gallbladder duplication is a rare anatomical variation with an incidence of approximately 1 in 3800 to 4000 live births, resulting from aberrant biliary organogenesis. This case report discusses a 35-year-old female who presented with intermittent lower abdominal pain, with initial imaging revealing a complex left adnexal cyst. Follow-up CT and ultrasound examinations revealed 2 distinct gallbladder-like structures in the gallbladder fossa, each with a cystic duct draining into the common bile duct.
