A 51-year-old male patient was receiving treatment for infection for approximately 10 years. However, as his condition gradually progressed to type II respiratory insufficiency, he was referred to our hospital, which was near his home. Computed tomography on his first visit revealed an abscess in the right lower lobe. Because respiratory insufficiency was evident, he was admitted the same day. We began treatment with meropenem, amikacin, and clarithromycin, but his symptoms did not improve. In accordance with the 2007 American Thoracic Society/Infectious Diseases Society of America statement, we administered linezolid, which resulted in gradual improvement in his physical status and imaging findings.

Preserved ratio impaired spirometry, airflow obstruction, and their trajectories in relationship to chronic kidney disease: a prospective cohort study.

Sci Rep

Department of Endocrinology, Beijing Tongren Hospital, Capital Medical University, No.2, Xihuan South Road, Beijing Economic and Technological Development Zone, Daxing District, Beijing, China.

Spirometry findings, such as restrictive spirometry and airflow obstruction, are associated with renal outcomes. Effects of spirometry findings such as preserved ratio impaired spirometry (PRISm) and its trajectories on renal outcomes are unclear. This study aimed to investigate the impact of baseline and trajectories of spirometry findings on future chronic kidney disease (CKD) events.

Growing evidence of the benefits of home ventilatory support in patients with chronic respiratory failure along with technological advances in ventilators have enabled their use in overly complex situations, shaping a new scenario for physicians. This has further given rise to new challenges related to their incorporation into current medical practice. However, this evolution needs to be coupled with knowledge and skills of physicians who are willing to prescribe Home Mechanical Ventilation (HMV), in order to prevent them from making inappropriate choices or adjustments that may ultimately have ethical and legal implications.
