Rhinocerebral mucormycosis is an acute, rapidly progressing, and life-threatening condition that predominantly affects individuals with uncontrolled diabetes and those who are immunocompromised. One critical complication of this disease is the thrombosis of orbital vessels, which can be indicative of angioinvasiveness and predict the subsequent development of cerebral infarctions. In this context, we present a case series of patients with rhino-orbital mucormycosis who experienced complications due to internal carotid artery thrombosis.

Acute sinusitis is a relatively common condition in pediatric patients. In most cases, it resolves completely with prompt medical management, but its rapid progression to severe orbital complications leading to restriction of extra-ocular movements and negative perception of light in the affected eye is rare and pose diagnostic and management challenges. The aggressive nature of acute rhinosinusitis requires urgent surgical intervention coupled with prompt medical management to prevent life threatening complications and outcomes.

A 78-year-old woman with untreated hypertension presented to the emergency department with a 2-day history of binocular diplopia, initially raising suspicion of neurological pathology. However, characteristic facial findings suggested sagging eye syndrome, confirmed by orbital magnetic resonance imaging. This case highlights the importance of clinicians paying close attention to distinctive facial features in patients with acute binocular diplopia.
