The fallacy of a roadmap computed tomography after an abdominal gunshot wound: A road that leads to nowhere.

J Trauma Acute Care Surg

November 2024

From the Program in Trauma, R Adams Cowley Shock Trauma Center, University of Maryland, Baltimore, Maryland.

Article Synopsis
  • - The study questions the value of preoperative abdominopelvic CT scans (CTAP) in patients with gunshot wounds to the abdomen, as current guidelines recommend them but their practical usefulness is being debated.
  • - Out of 149 patients analyzed, 72.5% showed clear signs for immediate surgical intervention (laparotomy), but CTAP findings only matched actual injury patterns in 57% of cases, missing significant injuries in 36.2% of patients.
  • - The results suggest that while CTAP might clarify unclear injury paths, it rarely changes management strategies for patients needing surgery and can lead to unnecessary operations in some cases.

Tracheal stenosis and paradoxical vocal fold motion are both common laryngological diagnoses that can present with similar symptoms of dyspnea. Co-morbid psychiatric issues can complicate diagnostic accuracy and lead to logical fallacies in the attribution of symptom etiology. We present a case of a 38-year-old female who presented repeatedly to the emergency department with respiratory distress, inspiratory stridor, wheezing, and anxiety.

Purpose: Cardiac transthyretin amyloidosis is a usually fatal form of restrictive cardiomyopathy for which clinical trials of treatments are ongoing. It is anticipated that quantitative nuclear medicine scintigraphy, which is experiencing growing interest, will soon be used to evaluate treatment efficacy. We investigated its utility for monitoring changes in disease load over a significant time period.

In this study, we aimed to measure glomerular filtration rate (mGFR) using Tc DTPA in patients with Child-Pugh C cirrhosis and normal serum creatinine levels; and to compare the performance of creatinine and cystatin C-based equations [estimated GFRs (eGFRs)] to TcDTPA GFR in the same group. We selected a group of 65 consecutive patients with advanced liver cirrhosis and apparently normal renal function by serum creatinine alone. Patients with confounding and reversible factors were excluded.

Background: The purpose of the study was to assess the accuracy of tibial and femoral component size prediction using computerised tomography (CT) based patient specific instruments in total knee arthroplasty.

Methods: Eighty-eight knees in 58 patients underwent total knee arthroplasty (TKA) using CT based patient specific instruments between March 2015 to April 2016. All patients were assessed for the pre operative femoral and tibial component sizes predicted by the CT-based pre-operative plan.
