Introduction: Mucinous appendiceal neoplasms are unique tumors in which >50 % of the tumor volume is composed of extracellular mucin. They may present as an unruptured mucin-filled appendix or, more commonly, with peritoneal metastases after rupture or transmural invasion of the primary tumor. Background: Wolffian adnexal tumor is a rare type of tumor that was first discovered and reported by Karim-inejad in 1973. Wolffian adnexal tumor lacks specific clinical manifestations and its histological morphology is similar to various other tumors, making it highly prone to misdiagnosis. To enhance our understanding of this disease, we hereby report a case of Wolffian adnexal tumor diagnosed and treated in our hospital.

Our case involves a 68-year-old postmenopausal patient with a history of total abdominal hysterectomy and right salpingo-oophorectomy performed 25 years ago. Introduction And Importance: Presentation of a giant para-ovarian cysts is rarely reported in the literature, with varying symptoms, methods of treatment, and complications. Herein, we highlight the diagnostic challenges faced in a low resource setting in the diagnosis of a giant para-ovarian cyst in a 17-year-old girl.

Case Presentation: A 17-year-old, virginal girl who presented with vague abdominal pain with an abdomino-pelvic mass of about 24 weeks pregnancy uterus size was diagnosed as a case of huge benign ovarian cyst with normal tumour markers.
