Background: Enteral nutrition is currently the route of choice for feeding critically ill patients with a functioning gut but delivery is commonly associated with disruptions. Common reasons for interruptions to enteral nutrition are fasting for diagnostic procedures, surgery and airway management. These interruptions result in significant calorie deficits that are associated with increased complications. We aimed to describe the specific interruptions in our patient group and the impact they have on nutrition delivery before and after implementation of a fasting guideline.
Methods: A service improvement project was undertaken over two different time points, 1 year apart, to evaluate the effectiveness of a fasting guideline in a general/trauma ICU in a London teaching hospital.
Results: There were 62 interruptions to enteral nutrition delivery with the first data collection and 64 in the second. Prolonged fasting before and after surgery and airway procedures were initially identified as the two most important causes of delays. Implementation of the fasting guideline resulted in statistical and clinical improvements in reducing fasting for airways procedures. The calorie deficit also statistically and clinically decreased as a result of the guideline.
Conclusions: We conclude that the introduction of a simple guideline stipulating reduced fasting times before ICU procedures can result in less time lost in feed interruptions and improved enteral nutrition delivery.

Objective: To gain insights into the experience, and impact, of using security staff to facilitate physical restraints for nasogastric tube feeding.
Design: A cross-sectional design using 39 individual interviews, three online focus groups and three written submissions involving young people with lived experience (PWLE), parents/carers, paediatric staff and security staff involved in nasogastric feeding under restraint in paediatric settings in England. Qualitative semistructured interviews were transcribed and thematically analysed.

Purpose: To evaluate and summarize the best evidence of home enteral tube feeding (HETF) care management in the elderly to provide an evidence-based basis for caregivers to implement care interventions.
Patients And Methods: Evidence on HETF care management in the elderly was retrieved from Chinese and international databases, guidelines, and websites of professional associations, including systematic reviews and expert consensuses, using the keywords of home enteral nutrition, home tube feeding, old, elder, home care, etc. The literature considered in this study was published from April 2019 to April 2024.

In recent years, the incidence of gastric cancer (GC) has been on the rise, surgical procedures usually require the removal of part of gastric tissue connected with the tumor lesion, which leads to poor postoperative health and adverse prognosis in patients. Probiotics, as an active microorganism, play an important role in improving gastrointestinal function and enhancing immunity. In this study, we randomized 135 GC patients into a control group, a probiotic group and a combination group.

Background: Gut microbiota disturbance may worsen critical illnesses and is responsible for the progression of multiple organ dysfunction syndrome. In our previous study, there was a trend towards a higher α-diversity of the gut microbiota in sequential feeding (SF) than in continuous feeding (CF) for critically ill patients. We designed this non-blinded, randomized controlled study to confirm these results.
