Article Abstract
Purpose Of Review: This paper will systemically review the risk of infections associated with current disease-modifying treatments and will discuss pre-treatment testing recommendations, infection monitoring strategies, and patient education.
Recent Findings: Aside from glatiramer acetate and interferon-beta therapies, all other multiple sclerosis treatments to various degrees impair immune surveillance and may predispose patients to the development of both community-acquired and opportunistic infections. Some of these infections are rarely seen in neurologic practice, and neurologists should be aware of how to monitor for these infections and how to educate patients about medication-specific risks. Of particular interest in this discussion is the risk of PML in association with the recently approved B cell depleting therapy, ocrelizumab, particularly when switching from natalizumab. The risk of infection in association with MS treatments has become one of the most important factors in the choice of therapy. Balance of the overall risk versus benefit should be continuously re-evaluated during treatment.

Objective: This study aims to adapt and provide psychometric support for the validation of version B of the Edinburgh Cognitive and Behavioural ALS Screen (ECAS) for the Portuguese population, addressing the need for consistent cognitive evaluations in amyotrophic lateral sclerosis (ALS). A second culturally adapted ECAS screen facilitates the accurate characterization of ALS progression, mitigates learning effects, and supports tailored care management.
Methods: The adaptation process included forward-backward translation, cultural adaptation, and cognitive debriefing on a prospective sample of 193 ALS patients and 106 controls.

As one of the most common neurodegenerative diseases, Multiple sclerosis (MS) is a chronic immune-driven disorder that affects the central nervous system (CNS). Due to the variety of symptoms, accurately diagnosing MS demands rigorous attention to differential diagnosis, as various disorders can closely mimic its clinical and paraclinical features. Although MR imaging techniques are gold standards in diagnosing MS, the feasibility of advanced Electroencephalogram (EEG) signal processing methods is discussed in this study to detect patients with MS disorder.

People with multiple sclerosis (PwMS) suffer cognitive and motor impairments that significantly impact their work life and daily functioning. Because virtual reality (VR) technologies have shown promise in the field of neurorehabilitation, in this study we developed a VR-based cognitive training program and assessed whether it could improve cognitive functioning in a cohort of PwMS (n = 33) and healthy control participants (n = 26). We observed that, in both groups of participants, repeated practice led to a day-by-day increase in correct responses and a progressive reduction in errors in the VR task.

Spinal cord sarcoidosis, an uncommon manifestation of neurosarcoidosis, presents diagnostic and therapeutic challenges because the condition is rare and has diverse clinical manifestations that can mimic other conditions such as multiple sclerosis and neuromyelitis optica spectrum disorder. A middle-aged African American female with a history of idiopathic intracranial hypertension and hydrocephalus with ventriculoperitoneal shunt presented with progressive, predominantly left-sided gait instability, weakness, and paresthesia. Cerebrospinal fluid showed lymphocytosis, red blood cells, elevated oligoclonal bands, and elevated kappa free light chains, concerning for multiple sclerosis.
