Headache disorders are among the most common presenting complaints in any neurology outpatient department. International Headache Society classifies headaches as "primary" or "secondary". The causes of secondary headaches are varied and intracranial hypotension is one of them. It typically presents clinically with postural headaches but most of the times, its diagnosis is delayed, as it is an uncommon cause and there is poor awareness among the medical fraternity about this condition. Imaging, especially magnetic resonance imaging (MRI), plays a crucial role in the diagnosis of intracranial hypotension by not only confirming the diagnosis but also detecting the cause in some cases. This case series tries to highlight the MRI findings of intracranial hypotension in three cases with different etiologies.

Among the causes of spontaneous intracranial hypotension (SIH), cerebrospinal fluid (CSF)-venous fistulas (CVFs) represent a recently discovered and poorly understood aetiology, for which invasive treatment (surgery vs endovascular treatment) may be required. However, CVFs are not always immediately identified during surgery. We describe a case of a woman with a 2-year history of SIH who was found to harbour a T8 CVF.

Iatrogenic intracranial hypotension is a known complication of spinal anesthesia that can lead to more severe conditions, such as dural or cerebral venous sinus thrombosis (CVST). This report presents a case of intracranial hypotension in a young woman after lumbar anesthesia for a cesarean section that was complicated by CVST and subsequently by lobar hemorrhage, clinically presenting with severe headache and seizures. The diagnosis was made via cerebral magnetic resonance (MR) imaging, and the patient was treated medically.

Aneurysmal Subarachnoid Hemorrhage (aSAH), resulting from ruptured aneurysms, is a major contributor to stroke-related mortality and morbidity. Despite advances in healthcare, aSAH remains severe and often leads to complications such as cerebral vasospasm (CV), cerebral infarction, and delayed ischemic neurological deficits (DIND). Clazosentan, an endothelin receptor antagonist, has demonstrated potential in alleviating vasospasm and its associated outcomes, although evidence of its efficacy remains unclear.
