This article presents a suboptimal control strategy with finite horizon for affine nonlinear discrete systems with both state and input delays. The Dynamic Programming Approach is used to obtain the suboptimal control sequence, but in order to avoid the computation of the Bellman functional, a numerical approximation of this function is proposed in every step. The feasibility of our proposal is demonstrated via an experimental test on a dehydration process and the obtained results show a good performance and behavior of this process. Then in order to demonstrate the benefits of using this kind of control strategy, the results are compared with a non optimal control strategy, particularly with respect to results produced by an industrial Proportional Integral Derivative (PID) Honeywell controller, which is tuned using the Ziegler-Nichols method.

Background: Tuberculosis (TB) diagnosis in the UK is impacted by delay and suboptimal culture-based microbiological confirmation rates due to the high prevalence of paucibacillary disease. We examine the real-world clinical utility of Xpert MTB/RIF Ultra (Xpert-Ultra) as a diagnostic test and biomarker of transmissible infection in a UK TB service.
Methods: Clinical specimens from suspected TB cases triple tested (smear microscopy, mycobacterial culture and Xpert-Ultra) at University Hospitals of Leicester NHS Trust (1 March 2018-28 February 2019) were retrospectively analysed.
WHO's 2013 PMTCT guidelines recommended lifelong antiretroviral therapy (ART) for HIV-infected pregnant and breastfeeding women, exclusive breastfeeding (EBF), nevirapine prophylaxis (NVP) and early infant diagnosis (EID) for HIV-exposed-breastfed infants. We examined the association between knowledge and adherence to these guidelines among 550 HIV-infected pregnant women in Maharashtra, India. Knowledge of PMTCT guidelines was assessed using a structured-questionnaire during enrollment.

Anaerobic digestion is a crucial process in wastewater treatment, renowned for its sustainable biogas production capabilities and the simultaneous reduction of environmental pollution. However, dysregulation of vital biological processes and pathways can lead to reduced efficiency and suboptimal biogas output, which can be seen through low counts per million of sequences related to three critical control points for methane synthesis. Namely, tetrahydromethanopterin S-methyltransferase (MTR), methyl-coenzyme reductase M (MCR), and CoB/CoM heterodisulfide oxidoreductase (HDR) are the last reactions that must occur.

Objective: Skin and soft tissue infections (SSTIs) account for over 2.8 million annual emergency department (ED) visits and often result in suboptimal antibiotic therapy. The objective of this study was to evaluate a set of interventions in minimizing inappropriate prescription of antibiotics for presumed SSTIs in the ED.

Routine preventive care (RPC) services are recommended for people with HIV, who have higher risk of certain preventable conditions. We used a pooled cross-section of patient-years to examine receipt of 5 annual RPC services among Medicaid enrollees in the US South. Data were person-level administrative claims (Medicaid Analytic eXtract, 2008-2012) and county-level characteristics for 16 Southern states plus District of Columbia.
