Shorter Premature Atrial Complex Coupling Interval Leads to Mechanical Dysfunction, Fibrosis, and AF in Swine.

JACC Clin Electrophysiol

October 2024

Section of Cardiac Electrophysiology, Division of Cardiology, University of California-San Francisco, San Francisco, California, USA. Electronic address:

Background: We have previously shown that dyssynchronous premature atrial complexes (PACs) from the lateral left atrium (LA) lead to greater atrial mechanical dysfunction, remodeling, and sustained atrial fibrillation (AF) than synchronous PACs from the interatrial septum. However, the impact of PAC coupling interval (CI) on atrial remodeling is unclear.

Objectives: This study sought to explore the effect of PAC CI on atrial mechanics and remodeling in the swine model.

Article Synopsis
  • * A full-term baby girl diagnosed with neonatal lupus showed symptoms like fetal arrhythmia and bradycardia, with heart irregularities linked to the presence of specific antibodies from her mother.
  • * The distinction between high-degree AV block and other issues like bigeminy PACs is crucial, as their management differs significantly; the case might also be the first documented instance of neonatal lupus presenting with atrial tachycardia.

Aims: Changes in myocardial mitochondrial morphology and function in premature ventricular contractions (PVCs)-induced cardiomyopathy (PVCCM) remain poorly studied. Here, we investigated the effects of PVCs with different coupling intervals (CIs) on myocardial mitochondrial remodelling in a canine model of PVCCM.

Methods And Results: Twenty-one beagles underwent pacemaker implantation and were randomised into the sham (n = 7), short-coupled PVCs (SCP, n = 7), and long-coupled PVCs (LCP, n = 7) groups.

Introduction: While pacing has been used for long QT syndrome (LQTs), the optimal pacing modality is controversial.

Case: We report a woman with bradycardia and a recently implanted single-chamber pacemaker experienced multiple syncope. No device dysfunction was found.

Article Synopsis
  • Tachy-brady syndrome results from sinus node dysfunction, leading to alternating arrhythmias, as seen in a 73-year-old male patient with significant mental health and medical issues who presented with symptoms like catatonia and paranoid delusions.
  • During his hospital stay, various arrhythmias were recorded, but the patient remained asymptomatic, confirming the diagnosis of tachy-brady syndrome through telemetry monitoring.
  • The treatment plan included beta-blockers and anticoagulation, and ultimately the patient received an implantable dual-chamber pacemaker to manage the risk of both bradyarrhythmias and tachyarrhythmias effectively.
