Objective: Voice dysfunction is frequently reported after thyroidectomy even in absence of vocal fold paralysis. The energy-based devices such as Harmonic scalpel (HS) or LigaSure (LS) are commonly used in conventional thyroidectomy. The objective of this study was to investigate the long-term voice outcome after total thyroidectomy using energy based devices.

Methods: Patients who underwent total thyroidectomy with or without central neck dissection for papillary thyroid carcinoma using HS or LS from April 2012 to March 2013 were involved. The videolaryngostroboscopy, acoustic analysis, voice range profile, voice perceptual evaluation, and voice handicap index-30 were obtained preoperatively, 1 week, 1 month, 3 months, 6 months, and 1year after thyroidectomy.

Results: Total 92 patients (HS group: 58 cases; LS group: 34 cases) were enrolled in this study. Demographics, tumor stage, and extent of operation were not significantly different between the HS and LS group. None of the patients evidenced any abnormalities at the pre- or postoperative videolaryngostroboscopic examination. The voice range profile (highest frequency), voice perceptual evaluation (grade, rough, and breathy), and voice handicap index-30 (total, functional, physical, and emotional) showed significantly worse scores in early postoperative period (<1month), but gradually returned to preoperative values. The all parameters of acoustic analysis, voice range profile, voice perceptual evaluation, and voice handicap index-30 were not significantly different between the HS and LS group.

Conclusion: The long-term voice change after thyroidectomy shows similar results regardless of the type of energy-based devices.

Time-dependent, patient-centered perceptions of quality measures for total joint arthroplasty: a cross-sectional, choice modeling study.

BMC Musculoskelet Disord

January 2025

VOICES Health Policy Research Center, Department of Orthopaedic Surgery, Stanford University, 450 Broadway Street MC: 6342, Redwood City, CA, 94603, USA.

Background: As value-based care arrangements continue to assess quality of care and costs, comprehensive and patient-centered definitions of quality of care are required. While patient-reported outcome measures are increasingly integrated into quality assessments following total joint arthroplasty (TJA), patient perceptions of quality paired with the phase of surgical care has not been described. The purpose of this study was to assess how TJA patients perceive measures of quality of care and assess if these perceptions change based on the phase of care.

Background: Conversational agents (CAs) are finding increasing application in health and social care, not least due to their growing use in the home. Recent developments in artificial intelligence, machine learning, and natural language processing have enabled a variety of new uses for CAs. One type of CA that has received increasing attention recently is smart speakers.

Objective: To compare the ototoxicity and survival in head and neck carcinoma patients treated with sequential (SEQ) and simultaneous integrated boost (SIB) of volumetric modulated arc therapy (VMAT).

Methods: This long-term prospective study enrolled patients with histologically confirmed head and neck carcinoma, all receiving VMAT treatment. Audiological assessments were done using various tests at baseline, two weeks, treatment completion, six months, and 12 months.

Simulation-based training in mental health is increasingly recognised as an effective tool within nursing education. This systematic review and meta-analysis aimed to evaluate the effectiveness of various simulation modalities, including standardised participants (SPs), role-plays, virtual reality (VR), mannequins and voice simulations, in improving educational outcomes for nursing students. A comprehensive literature search was conducted to identify studies evaluating the impact of mental health simulation on nursing education.
