A 10-month-old male Welsh Corgi with a history of acute blindness underwent neuro-ophthalmological testing and magnetic resonance imaging (MRI). Vision testing revealed complete visual deficits but the electroretinograph and pupillary light reflex were normal in both eyes. The motor and sensory functions of the eyelids and eyes were also normal. The MRI revealed compression of the optic chiasm caused by severe ventriculomegaly in the lateral and third ventricles. Such lesions are associated with inflammatory stenotic lesions in the mesencephalic aqueduct. Moderate neutrophilic pleocytosis was observed during cerebrospinal fluid analysis and Acinetobacter lwoffii was isolated, leading to a diagnosis of Acinetobacter-positive obstructive hydrocephalus. This is the first reported case of culture-proven Acinetobacter-associated postencephalitic hydrocephalus with acute blindness in a dog.

Background: Optic neuritis (ON) is a complex clinical syndrome that has diverse etiologies and treatments based on its subtypes. Notably, ON associated with multiple sclerosis (MS ON) has a good prognosis for recovery irrespective of treatment, whereas ON associated with other conditions including neuromyelitis optica spectrum disorders or myelin oligodendrocyte glycoprotein antibody-associated disease is often associated with less favorable outcomes. Delay in treatment of these non-MS ON subtypes can lead to irreversible vision loss.

Leber's hereditary optic neuropathy (LHON) is an ocular mitochondrial disease that involves the impairment of mitochondrial complex I, which is an important contributor to blindness among young adults across the globe. However, the disorder has no available cures, since the approved drug idebenone for LHON in Europe relies on bypassing complex I defects rather than fixing them. Herein, mRNA-loaded nanoparticle (mNP)-engineered mitochondria (mNP-Mito) were designed to replace dysfunctional mitochondria with the delivery of exogenous mitochondria, normalizing the function of complex I for treating LHON.
