Posterior glenohumeral instability is an increasingly important clinical finding in athletic patients. Over the last decade, basic and clinical research has improved our understanding of the pathoanatomy and biomechanics of this challenging disorder, as well as our ability to diagnose and appropriately treat it. Although recurrent posterior shoulder instability is not as common as anterior instability, it is prevalent among specific populations, including football and rugby players, and may be overlooked by clinicians who are unaware of the typical physical examination and radiographic findings.

This study aimed to (1) examine the acute changes in the glenohumeral range of motion (ROM) after repetitive pitching and (2) clarify whether arm speed during pitching is associated with changes in the glenohumeral internal rotation (IR) and horizontal adduction (HADd) ROM. Fifteen healthy college males with over five years of baseball experience participated. Glenohumeral ROMs of IR, external rotation, and HADd were measured using a digital inclinometer before, immediately after, and one day after completing 100 repetitive full-effort pitches.

A 25-year-old male presented with a ballistic fracture of the right glenoid resulting in > 30% loss of the posterior glenoid articular surface and acute posterior glenohumeral instability that was treated with open reduction internal fixation with iliac crest autograft transfer. There is limited consensus on the operative management of ballistic intra-articular fractures due to the heterogeneity of these injuries. Acute posterior glenohumeral instability secondary to a ballistic fracture is a rare injury pattern.

Introduction: Reverse shoulder arthroplasty (RSA) and anatomic total shoulder arthroplasty (TSA) are well-known methods of treating glenohumeral arthritis, which often leads to posterior wear of the glenoid. This study compared minimum two-year outcome measures in patients treated with RSA and TSA for Walch B2 and B3 glenoids.
Methods: Thirty-eight shoulders underwent TSA and 40 shoulders underwent RSA by two fellowship-trained shoulder surgeons at a tertiary referral center.
