Skin GVHD Mimicking Betadine Rash in a Post Allogeneic Transplant Patient.

A 32-years-old male developed exfoliating skin lesions in bilateral axillae and groins on D-42 post matched sibling allogeneic transplant. These lesions masqueraded betadine burns with a positive history of betadine application in the affected regions. He gradually developed lesions involving other parts of the body with Glucksberg grade-IV acute GVHD (skin-stage: IV/gut-stage: III). He succumbed to his illness. We present this case to highlight the rare initial presentation of skin GVHD involving localized body regions with sparing of rest of the body and mimicking betadine burns.

We report a case of Acanthamoeba infection in an HCT recipient with steroid-refractory GVHD. We highlight the multiple challenges that free-living ameba infections present to the clinician, the clinical laboratory, transplant infectious disease for review, hospital epidemiology if nosocomial transmission is considered, and public health officials, as exposure source identification can be a significant challenge. Transplant physicians should include Acanthamoeba infections in their differential diagnosis of a patient with skin, sinus, lung, and/or brain involvement.

Background: Allogeneic transplant for patients with transfusion-dependent thalassemia is challenging once there has been iron overload and chronic transfusion support.

Objective(s): A transplant strategy that reduced intensity of the preparative regimen and tailored immunosuppression to both support donor engraftment and prevent GVHD was developed for this population. The combination of a pretransplant immunosuppression phase with reduced dosing of fludarabine/prednisone, treosulfan-based preparative regimen with reduced cyclophosphamide dosing, and introduction of a calcineurin/methotrexate-free GVHD prophylaxis/engraftment supporting regimen with abatacept/sirolimus/ATG was tested.
