This work presents an observational scale which takes into account different observable indicators of discomfort of patients in end-of-life situations with difficulties in communicating due to cognitive impairment, fatigue or sedation and provides details of its validation. In all, 71 adults participated. Model fit was adequate ((27) = 43.28,  = .024, comparative fit index = .975, root mean square error of approximation = .092 and confidence interval 90% (.033-.140)). Alpha coefficient was .70 and composite reliability index was .90. Our study provides data regarding the properties of a discomfort assessment scale. Such a scale is needed and could be very useful for the evaluation of such patients and thus to attend to their needs.

We carried out the health situation analysis in the Legal Amazon through morbidity and mortality indicators and the comparison between intra and inter-state federation of the region and Brazil. Analysis of the health situation, trends, and identification of clusters in the Brazilian Amazon, for the period from 2010 to 2021, using secondary data available in official health information systems. Inspired by Dana Birksted-Breen's ideas on reverberation time, the author explores the changeability and transformation of the sensations of time and space and their connection to early embodied phantasies in the treatment of a 10-year-old boy. The experience of time changes (summarized under "time elasticity" to reflect the various forms this can take) is lived out in the transference relationship from the beginning of the therapeutic encounter. The author proposes the simultaneous development of the capacity to accept "objective" time, the establishment of a tri-dimensional space within the self and between objects and tolerating separateness and separation.
