Takotsubo cardiomyopathy, also described as apical ballooning syndrome/stress-induced cardiomyopathy, imitates acute coronary syndrome and is usually related to a massive physiological or emotional stressor. We describe perhaps the first reported case to the best of our knowledge of a 55-year-old Caucasian woman who presented with congestive heart failure after having a regadenoson cardiac stress test a few hours prior to presentation to the hospital. Transthoracic echocardiogram revealed reduced heart function. She had normal coronaries on cardiac catheterisation, and left ventriculography confirmed apical ballooning syndrome. She underwent guideline-directed therapy, and heart function improved in the repeat echocardiogram along with clinical resolution of symptoms.

Background: Estimation of coronary artery calcification (CAC) can provide important prognostic information in patients with suspected coronary artery disease.
Objectives: The authors aimed to investigate whether CAC evaluation offers additional insight into the association of stress electrocardiographic (ECG) changes with adverse outcomes in patients undergoing myocardial perfusion imaging (MPI).
Methods: Twelve thousand two hundred sixty-five patients (47% female, median: 64 (56-73) years, body mass index: 29 ± 7 kg/m) who underwent exercise treadmill or regadenoson single-photon emission computed tomography/computed tomography (CT) MPI during June 2016 to May 2022 at a large academic center were retrospectively studied.

Coronary computed tomography angiography (CCTA) is a powerful tool to rule out coronary artery disease (CAD). In the last decade, myocardial perfusion CT (CTP) technique has been developed for the evaluation of myocardial ischemia, thereby increasing positive predictive value for diagnosis of obstructive CAD. A diagnostic strategy combining CCTA and perfusion acquisitions provides both anatomical coronary evaluation and functional evaluation of the stenosis, increasing the specificity and the positive predictive value of cardiac CT.
