Idiopathic Tumoral Calcinosis - Rare Clinico Pathological Entity: A Report of Two Cases.

Tumoral calcinosis is a rare disease reported mainly in blacks of tropical and subtropical African regions. It is characterized by tumour-like periarticular deposits of calcium that are found in the regions of the hip, shoulder, elbow and small joints of hands and feet. The involvement of this disease in a healthy patient is extremely rare in young and middle aged individuals. We are presenting two cases with large, painless periarticular swellings in subscapular joint and feet. First case presented with recurrent swelling in different anatomical locations at different time intervals involving the hip and followed by shoulder joint. Second case presented with swelling around the ankle joint. These two cases were diagnosed as Idiopathic tumoral calcinosis based on biochemical, radiological and histopathological findings.

Calcinosis, Raynaud's phenomenon, esophageal dysmotility, sclerodactyly, and telangiectasia (CREST) syndrome, a systemic sclerosis subtype, features skin thickening, vascular issues, and organ involvement, causing complications in the gastrointestinal and musculoskeletal systems. A woman in her 70s with hypertension, breast cancer and diverticulosis underwent laparoscopic anterior resection for a tubule-villous adenoma, converted to open Hartmann's with aorto-bi-iliac bypass due to a vascular injury. Intraoperative complications included haem-o-lok penetration of the calcified aorta, necessitating vascular team intervention. Postoperative issues included bilateral popliteal artery emboli requiring embolectomy and fasciotomy, and a parastomal abscess.

It is urgent for patients with chronic kidney disease (CKD) to develop a robust and facile therapy for effective control of serum phosphate and reasonable regulation of gut microbiota, which are aiming to prevent cardiovascular calcification and reduce cardiovascular complications. Here, bioinspired by intestinal microstructures, we developed biomimetic wrinkled prebiotic-containing microspheres with enhanced intestinal retention and absorption for reducing hyperphosphatemia and vascular calcification of CKD model rats. The resultant CSM@5 microspheres exhibited favorable phosphate binding capacity in vitro and could effectively reduce serum concentration of phosphorous in vivo.
