Purpose: To assess effectiveness of a -blocker bisoprolol basing on a study of variability of cardiac rhythm and polymorphism of -1 adrenergic receptor gene (ADRB1).

Material And Methods: We examined 99 patients with coronary atherosclerosis either with stable angina without history of vascular events or more than 6 month after myocardial infarction and/or coronary intervention.

Results And Conclusion: Patients with coronary atherosclerosis including those after myocardial infarction had signs of autonomic dysfunction regardless of the presence of genetic polymorphism Gly389Arg ADRB1. Sympathetic influences at orthostatic test performed during taking bisoprolol were more pronounced in patients with Gly389Gly ADRB1 genotype than in patients with Gly389Arg ADRB1 genotype. Basing on this observation we conclude that the control by bisoprolol of heart rate regulation in patients with Gly389Gly ADRB1 genotype should be considered inadequate.

Pulmonary rehabilitation (PR) services are increasingly using alternative programme delivery modes, for example telerehabilitation strategies including videoconferencing, to improve patient choice and accessibility. Although telerehabilitation results in improvements in core outcomes, the effect on knowledge attainment is not known. To observe the real-world responses of patients choosing to undergo videoconference PR to a matched control group choosing to undergo in-person PR, in terms of knowledge attainment.

Background: Vascular calcification is a detrimental aging pathology markedly accelerated in patients with chronic kidney disease. Prelamin A is a biomarker of vascular smooth muscle cell aging that accelerates calcification however the mechanisms remain undefined.

Methods: Vascular smooth muscle cells were transduced with prelamin A using an adenoviral vector and epigenetic modifications were monitored using immunofluorescence and targeted polymerase chain reaction array.

Some patients with aortic stenosis may require multiple valve interventions in their lifetime, and choosing transcatheter aortic valve replacement (TAVR) as the initial intervention may be appealing to many. If their transcatheter heart valve degenerates later in life, most will hope to undergo redo-TAVR. However, if redo-TAVR is not feasible, some may have to undergo surgical explantation of their transcatheter heart valve (TAVR-explant).

Background: Plasma NT-proBNP (N-terminal pro-B-type natriuretic peptide) is commonly used to diagnose heart failure with preserved ejection fraction (HFpEF), but its diagnostic performance in the ambulatory/outpatient setting is unknown because previous studies lacked objective reference standards.

Methods: Among patients with chronic dyspnea, diagnosis of HFpEF or noncardiac dyspnea was determined conclusively by exercise catheterization in a derivation cohort (n=414), multicenter validation cohort 1 (n=560), validation cohort 2 (n=207), and a nonobese Japanese validation cohort 3 (n=77). Optimal NT-proBNP cut points for HFpEF rule out (optimizing sensitivity) and rule in (optimizing specificity) were derived and tested, stratified by obesity and atrial fibrillation.
