A technique for posterior meniscal root reinsertion is presented. With the arthroscope in the central transtendinous portal for a better view, a 5-mm transtibial tunnel is created with the aid of an anterior cruciate ligament guide open to 45°. A suture device, which consists of a long needle with an eyelet on its tip, is introduced through the tunnel with a suture thread inserted through the eyelet, while the meniscus is stabilized with a grasper inserted through the anterior portal. The meniscus is pierced with the device, and the suture thread is recovered with said grasper. A finger-tip pincer is inserted through the tunnel to recover the thread. The same procedure is followed to perform a second stitch. A lasso loop is made for both stitches, and the resultant tails are knotted to a knotless suture anchor, which is inserted in the anterior cortex of the tibia, 1 cm distal to the extra-articular end of the tibial tunnel.

Background: Anterior cruciate ligament (ACL) injury often leads to posttraumatic osteoarthritis (PTOA), despite ACL reconstruction (ACLR). Medial meniscal extrusion (MME) is implicated in PTOA progression but remains understudied after ACL injury and ACLR.
Hypothesis/purpose: It was hypothesized that MME would increase longitudinally after ACL injury and ACLR, with greater changes in the ipsilateral knee compared with the contralateral knee, leading to cartilage degeneration.

Purpose: To compare graft remodeling, as measured by magnetic resonance imaging (MRI), and clinical outcomes between patients who underwent isolated anterior cruciate ligament reconstruction (ACLR) versus combined anterior cruciate ligament and anterolateral ligament reconstruction (ACLR + ALLR).
Methods: A retrospective review was conducted on patients who underwent primary ACLR with quadruple hamstring grafts between January 2019 and March 2022, with a minimum follow-up period of 2 years. Patients were categorized into two groups based on the addition of ALLR with tibialis anterior allografts: an isolated ACLR group and an ACLR + ALLR group.

All-inside techniques are based on devices that use PEEK (polyether ether ketone) or biocomposite anchors placed at extracapsular locations such as anchorage points over which the sutures are tied. However, because of complications like irritability and intra-articular migration of these hard anchors, suture-based all-inside meniscal repair systems are now gaining popularity. Although these devices have advantages over conventional all-inside devices, they are costly, thus limiting their widespread use.
