Myxoma is a rare cause of severe mitral stenosis. We describe a challenging case of severe mitral stenosis and reversible pulmonary hypertension produced by a giant left atrial myxoma. The greatest risk is a diagnostic delay due to focus on discarding a pulmonary etiology of dyspnea. Physical examination and early echocardiographic assessment are the keys to avoid overlooking this uncommon pathology.

Heart failure due to acute mitral regurgitation (MR) may present with atypical findings on physical examination and transthoracic echocardiography (TTE), making the diagnosis difficult. A 72-year-old male was transferred to our hospital with severe cardiorespiratory failure and was initially suspected to have severe pneumonia based on the acute onset and imaging findings. The patient was eventually diagnosed with acute heart failure secondary to MR.

With a prevalence of 2-3% in the general population, mitral valve prolapse (MVP) is the most common valvular heart disease. The clinical course is benign in the majority of patients, although severe mitral regurgitation, heart failure, and sudden cardiac death affect a non-negligible subset of patients.
