Aim: To clarify clinical and pathogenetic predictors of outcome in acute period of atherothrombotic stroke (ATS).

Material And Methods: One hundred and nineteen patients with ATS were examined. Characteristics of the clinical picture, a number of white blood cells in peripheral blood in the stage of apoptosis, necrosis as well as the state of intracellular antioxidant protective system were studied.

Results: Among the clinical factors that can worsen the course of acute period of ATS, the most important are the size of the focus, consciousness disorders in the first day, basal systolic blood pressure, age of the patient (r=0.496; p=0.01 on the 1st day, r=0.739; p<0.0001 on the 7th day and r=0.620, p<0.0001on the 14th day). A direct influence of mitochondrial dysfunction and mitochondrial-induced apoptosis on the course of ATS in the acute period was shown. There was a direct correlation between the severity of ATS and quantity of Mito + cells on the 1st (r=0.742; p=0.009) and on the 7th day (r=0.717; p=0.002). The correlation was revealed between the severity of ATS and content of ANV+-white blood cells on the 7th day (r=0.595; p=0.015) and on the 14th day (r=0.670; p=0.007). Negative correlation was found between the activity of total SOD and Mn-SOD on the 1st day and the severity of ATS on the 7th day (r=0.628; p=0.010) and (r=0.675; p=0.008), respectively. The last finding indicates the prognostic value of these parameters.

Objective: A significant number of patients with chronic thromboembolic pulmonary hypertension (CTEPH) are not eligible for pulmonary endarterectomy and may be treated with balloon pulmonary angioplasty (BPA). Although BPA programs have recently been developed in Brazil, no results have yet been published. The objective of this study was to assess the clinical and hemodynamic progression of the first patients treated with BPA at our center.

Aim: Air pollution remains the single largest environmental health risk factor, while atrial fibrillation (AF) is the most prevalent arrhythmia globally. The study aimed to investigate the relationship between short-term exposure to air pollution and acute AF admissions.

Methods: Individual data on AF hospitalization in the years 2011-2020 were collected from the National Health Fund in Poland (ICD-10: I48.

Importance: The emergency department (ED) offers an opportunity to initiate palliative care for older adults with serious, life-limiting illness.

Objective: To assess the effect of a multicomponent intervention to initiate palliative care in the ED on hospital admission, subsequent health care use, and survival in older adults with serious, life-limiting illness.

Design, Setting, And Participants: Cluster randomized, stepped-wedge, clinical trial including patients aged 66 years or older who visited 1 of 29 EDs across the US between May 1, 2018, and December 31, 2022, had 12 months of prior Medicare enrollment, and a Gagne comorbidity score greater than 6, representing a risk of short-term mortality greater than 30%.

Importance: There have been limited evaluations of the patients treated at academic and community hospitals. Understanding differences between academic and community hospitals has relevance for the design of clinical models of care, remuneration for clinical services, and health professional training programs.

Objective: To evaluate differences in complexity and clinical outcomes between patients admitted to general medical wards at academic and community hospitals.

Background: Proprotein convertase subtilisin/kexin type 9 (PCSK9) monoclonal antibodies (mAbs) have demonstrated promising effects in lowering cardiovascular incidents among patients with acute coronary syndrome. However, their influence on early platelet reactivity after primary percutaneous coronary intervention (PPCI) remains unclear.

Objectives: This research sought to investigate the effects of entirely human anti-PCSK9 antibodies on platelet function as measured by thrombelastography and 12-month postoperative results in patients receiving PPCI and treated with ticagrelor-based dual antiplatelet therapy.
