Three hundred and fifty-eight patients undergoing clean orthopaedic surgery were included in this prospective study. The knives used during surgery, i.e. the knife used for skin incision (the skin-knife) and the knife used to finish the operation were bacteriologically examined. We found no evidence that the practise of discarding the knife after skin incision reduced the incidence of wound infection after clean orthopaedic surgery. Laminar air flow significantly reduced the rate of knife contamination.

Bolstered bone regeneration by multiscale customized magnesium scaffolds with hierarchical structures and tempered degradation.

Addressing irregular bone defects is a formidable clinical challenge, as traditional scaffolds frequently fail to meet the complex requirements of bone regeneration, resulting in suboptimal healing. This study introduces a novel 3D-printed magnesium scaffold with hierarchical structure (macro-, meso-, and nano-scales) and tempered degradation (microscale), intricately customized at multiple scales to bolster bone regeneration according to patient-specific needs. For the hierarchical structure, at the macroscale, it can feature anatomic geometries for seamless integration with the bone defect; The mesoscale pores are devised with optimized curvature and size, providing an adequate mechanical response as well as promoting cellular proliferation and vascularization, essential for natural bone mimicry; The nanoscale textured surface is enriched with a layered double hydroxide membrane, augmenting bioactivity and osteointegration.

Surgeons frequently use allograft bone due to its osteoconductive, osteoinductive, and osteogenic properties. Preservation processes are employed to clean the allograft, improve its conservation, and ensure its sterilization. Many current processes use the properties of supercritical CO to remove bone marrow.

Total Knee Arthroplasty Automated Implant Detector: An Uncertainty-Aware Deep Learning Classifier to Identify Total Knee Arthroplasty Implants.

Background: A drastic increase in the volume of primary total knee arthroplasties (TKAs) performed nationwide will inevitably lead to higher volumes of revision TKAs in which the primary knee implant must be removed. An important step in preoperative planning for revision TKA is implant identification, which is time-consuming and difficult even for experienced surgeons. We sought to develop a deep learning algorithm to automatically identify the most common models of primary TKA implants.

Regulation of Bone Remodeling by Metal-Phenolic Networks for the Treatment of Systemic Osteoporosis.

Osteoporosis is a systemic metabolic disease that impairs bone remodeling by favoring osteoclastic resorption over osteoblastic formation. Nanotechnology-based therapeutic strategies focus on the delivery of drug molecules to either decrease bone resorption or increase bone formation rather than regulating the entire bone remodeling process, and osteoporosis interventions suffer from this limitation. Here, we present a multifunctional nanoparticle based on metal-phenolic networks (MPNs) for the treatment of systemic osteoporosis by regulating both osteoclasts and osteoblasts.
