Background: The chronic effects of ST-segment elevation myocardial infarction (STEMI) on the atrioventricular conduction (AVC) system have not been elucidated. This study aimed to evaluate the incidence, predictors, and outcomes of worsened AVC post-STEMI in patients treated with a primary percutaneous coronary intervention (PCI).

Patients And Methods: The current analysis included patients from the HORIZONS-AMI trial who underwent primary PCI and had available ECGs. Patients with high-grade atrioventricular block or pacemaker implant at baseline were excluded.

Results: Analysis of ECGs excluding the acute hospitalization period indicated worsened AVC in 131 patients (worsened AVC group) and stable AVC in 2833 patients (stable AVC group). Patients with worsened AVC were older, had a higher frequency of hypertension, diabetes, renal insufficiency, previous coronary artery bypass grafting, and predominant left anterior descending culprit lesions. Predictors of worsened AVC included age, hypertension, and previous history of coronary artery disease. Worsened AVC was associated with an increased rate of all-cause death and major adverse cardiac events (death, myocardial infarction, ischemic target vessel revascularization, and stroke) as well as death or reinfarction at 3 years. On multivariable analysis, worsened AVC remained an independent predictor of all-cause death (hazard ratio: 2.005, confidence interval: 1.051-3.827, P=0.0348) and major adverse cardiac events (hazard ratio 1.542, confidence interval: 1.059-2.244, P=0.0238).

Conclusion: Progression of AVC system disease in patients with STEMI treated with primary PCI is uncommon, occurs primarily in the setting of anterior myocardial infarction, and portends a high risk for death and major adverse cardiac events.

Article Synopsis
  • - The study investigates the effects of cystic fibrosis transmembrane conductance regulator (CFTR) modulator therapies on health-related quality of life (HRQoL) and well-being among people with cystic fibrosis (PwCF) and their caregivers.
  • - Results show that adult PwCF currently on modulator therapy experience better physical and mental health compared to those who have stopped or never used the treatments, but some (23%) report declines in mental well-being.
  • - The findings aim to inform future research, policy, and communication efforts that could enhance the well-being of PwCF by understanding their experiences with modulator therapies.

Article Synopsis
  • The study focused on the prevalence and impact of mitral regurgitation (MR) and tricuspid regurgitation (TR) in patients with cardiac amyloidosis (CA), highlighting a lack of existing research on this topic.
  • Among 538 patients analyzed, 44.6% had no significant MR/TR, while a significant portion experienced varying degrees of MR or TR, with common causes being atrial functional issues and right ventricular overload.
  • Results indicated that patients with MR/TR had worse health outcomes, including higher hospitalization rates for heart failure, more symptoms, and increased risk of death or worsening heart failure, with TR presenting the most severe risks.

Article Synopsis
  • * A review of 95 patients revealed that higher AVC scores and volumes were associated with increased severity of concomitant AR, with significant differences observed across the groups (no AR, mild AR, moderate AR).
  • * The findings suggest that AVC severity is correlated with the degree of AR in patients with severe AS, indicating that AVC may contribute to the worsening of AR and overall disease condition.

Background And Objectives: Evidence regarding the safety and efficacy of messenger RNA (mRNA) vaccines in patients with myasthenia gravis (MG) after immunosuppressive therapies is scarce. Our aim is to determine whether the mRNA-1273 vaccine is safe and able to induce humoral and cellular responses in patients with MG.

Methods: We performed an observational, longitudinal, prospective study including 100 patients with MG of a referral center for MG in our country, conducted from April 2021 to November 2021 during the vaccination campaign.

In-depth understanding of pathophysiological processes occurring in the vasculature of the equine distal limb is of great importance to improve both diagnostic and therapeutic approaches to diseases. To gain further insights, a model allowing high-resolution 3D-visualization of the vasculature is necessary. This pilot study evaluated the feasibility of restoring vascular perfusion in frozen-thawed distal equine cadaver limbs without prior preparation using computer tomographic imaging (CT).
