Cephalic tetanus (CT) is a severe form of tetanus that follows head wounds and the intoxication of cranial nerves by tetanus neurotoxin (TeNT). Hallmarks of CT are cerebral palsy, which anticipates the spastic paralysis of tetanus, and rapid evolution of cardiorespiratory deficit even without generalized tetanus. How TeNT causes this unexpected flaccid paralysis, and how the canonical spasticity then rapidly evolves into cardiorespiratory defects, remain unresolved aspects of CT pathophysiology.

Tetanus is a serious disease that has become rare in countries that adopt an effective and sustainable vaccination strategy; however, tetanus remains a fairly common problem in developing countries. The diagnosis of tetanus is fairly easy. However, its cephalic form is a rare but potentially life-threatening neurological condition caused by the bacteria , which can result in symptoms such as spasms, rigidity, and paralysis of various muscles and nerves in the head and neck region.

Article Synopsis
  • Tetanus, though rare in developed countries like Japan, poses a diagnostic challenge due to its varied symptoms and lack of awareness among medical professionals and patients.
  • This study analyzed 10 patients with tetanus over a decade at a tertiary hospital, noting that common initial symptoms included neck stiffness, dysarthria, and dysphagia, with most patients initially visiting departments such as dental and otorhinolaryngology.
  • Only two patients were diagnosed with tetanus at their first visit, highlighting the confusion in symptom attribution between central nervous system disorders and oral/dental issues, indicating a need for further research.
