A 26-year-old female presented with 4-years duration of asymptomatic spots on her left leg. She was otherwise healthy. On examination several purpuric patches up to 3 cm. in diameter were found on the flexure aspect of her left leg. Histological examination revealed lymphocytic infiltrate, with mild atypia, with red blood cell extravasation and hemosiderin deposition in the papillary dermis. The clinical and histological findings were compatible with the diagnosis of early stage purpuric mycosis fungoides (MF). Purpuric form of MF is a rare form of the disease. Physician should distinguish this disease from benign forms of pigmented purpuric dermatoses.

Introduction: Soft tissue defect in the lower limb presents as a difficult reconstructive challenge. Cross-leg flap was routinely used in the past for the salvage of the lower limb but is seldom used nowadays due to advances in microsurgical procedures.
Case Presentation: We present a case of an 18-year-old male who presented with a complex soft tissue defect of 25 × 10 cm on the anterolateral aspect of the right leg following a motor vehicle accident.

Acrometastasis is an extremely rare diagnosis, invariably associated with poor prognosis. A 60-year-old female with complaints of cough and breathing difficulty also presented with pain and swelling in her left leg. Radiological investigations suggested a double primary in the lung and leg; histopathology and immunohistochemistry (IHC) confirmed the lesion in the leg to be metastatic from the lung primary.

The anterolateral thigh (ALT) flap is a commonly used donor site for reconstructive surger, especially in head and neck reconstructions. The flap's success is primarily determined by the quality and quantity of its perforating vessels, which is why clinicians typically prefer harvesting from a healthy leg with intact vascular anatomy. Poliomyelitis typically causes unilateral lower limb paralysis, resulting in muscle atrophy and deformities.
