Background/aims: The present study aimed to evaluate the safety and efficacy of simeprevir-based triple therapy with reduced doses of pegylated interferon (PEG-IFN) and ribavirin for interferon (IFN) ineligible patients, such as elderly and/or cirrhotic patients, and to elucidate the factors contributing to a sustained virologic response (SVR).
Methods: One hundred IFN ineligible patients infected with genotype 1b hepatitis C virus (HCV) were treated. Simeprevir (100 mg) was given orally together with reduced doses of PEG-IFN-α 2a (90 μg), and ribavirin (200 mg less than the recommended dose).
Results: The patients' median age was 70 years, and 70 patients were cirrhotic. Three patients (3%) discontinued treatment due to adverse events. The SVR rate was 64%. Factors that significantly contributed to the SVR included the γ-glutamyl transferase and α-fetoprotein levels, interleukin- 28B (IL28B) polymorphism status, and the level and reduction of HCV RNA at weeks 2 and 4. The multivariate analysis showed that the IL28B polymorphism status was the only independent factor that predicted the SVR, with a positive predictive value of 77%.
Conclusions: Simeprevir-based triple therapy with reduced doses of PEG-IFN and ribavirin was safe and effective for IFN ineligible patients infected with genotype 1b HCV. IL28B polymorphism status was a useful predictor of the SVR.

Despite decades of improvements in cytotoxic therapy, the current standard of care for locally advanced pancreatic cancer (LAPC) provides, on average, only a few months of survival benefit. Stereotactic Body Radiation Therapy (SBRT), a technique that accurately delivers high doses of radiation to tumors in fewer fractions, has emerged as a promising therapy to improve local control of LAPC; however, its effects on the tumor microenvironment and hypoxia remain poorly understood. To explore how SBRT affects pancreatic tumors, we combined an orthotopic mouse model of pancreatic cancer with an intravital microscopy platform to visualize changes to the in vivo tumor microenvironment in real-time.

Oxidative stress and inflammation are indispensable components of ischemia-reperfusion (IR) injury. In this study, we investigated the effects of low and high doses of caftaric acid (CA) on reducing kidney and remote organ damage induced by IR. We divided Wistar rats into four groups: sham, IR, low (40 mg/kg body weight (BW)), and high (80 mg/kg BW) CA groups.

Background: Excessive submental fat under the chin is a known aesthetic concern because of its negative impact on facial appearance and psychological well-being. AYP-101 is a newly developed injectable agent containing 93% soybean phosphatidylcholine (SPC) designed to reduce submental fat. We conducted a phase 1 study to evaluate the safety, pharmacokinetic (PK), and lipid profile effects of AYP-101.

Background: Recent studies show that the increase in breath hydrogen (BH) and symptoms after ingestion of inulin is reduced by co-administering psyllium.
Objectives: To determine if slowing delivery of inulin to the colon by administering it in divided doses would mimic the effect of psyllium. Primary endpoint was the BH area under the curve AUC.

Background: Plant-based foods have reduced protein digestibility and frequently display unbalanced amino acid profiles. Plant-based foods are therefore considered inferior to animal-based foods in their anabolic potential. No study has assessed the anabolic potential of a vegan diet that provides a large variety of plant-based protein sources in older adults.
