Peripheral arterial disease (PAD) is a clinical manifestation of systemic atherosclerosis and is associated with significant morbidity and mortality. Vascular physicians come across PAD of different complexity and among them complex total occlusions present a formidable challenge to endovascular operators. Newer tools and techniques, particularly in the retrograde approach, have made endovascular therapy applicable to increasingly complex disease. In this article, we review various interventional devices and techniques that are key to achieving success in complex anatomic subsets.

Effective road terrain recognition is crucial for enhancing the driving safety, passability, and comfort of autonomous vehicles. This study addresses the challenges of accurately identifying diverse road surfaces using deep learning in complex environments. We introduce a novel end-to-end Tire Noise Recognition Residual Network (TNResNet) integrated with a time-frequency attention module, designed to capture and leverage time-frequency information from tire noise signals for road terrain classification.

Self-propelled micro/nanomotors (MNMs) represent a groundbreaking advancement in precision drug delivery, offering potential solutions to persistent challenges such as systemic toxicity, limited bioavailability, and nonspecific distribution. By transforming various energy sources into mechanical motion, MNMs are able to autonomously navigate through complex physiological environments, facilitating targeted delivery of therapeutic agents to previously inaccessible regions. However, to achieve efficient in vivo drug delivery, biomedical MNMs must demonstrate their ability to overcome crucial physiological barriers encompassing mucosal surfaces, blood flow dynamics, vascular endothelium, and cellular membrane.

Hypertrophic cardiomyopathy (HCM) is a complex and heterogeneous cardiac disorder, often complicated by cardiogenic shock, a life-threatening condition marked by severe cardiac output failure. Managing cardiogenic shock in HCM patients presents unique challenges due to the distinct pathophysiology of the disease, which includes dynamic left ventricular outflow tract obstruction, diastolic dysfunction, and myocardial ischemia. This review discusses current and emerging therapeutic strategies tailored to address the complexities of HCM-associated cardiogenic shock and other diseases with similar pathophysiology that provoke left ventricular outflow tract obstruction.
