Congenital Gerbode defects, consisting of a deficiency in the membranous septum causing left ventricle-to-right atrium shunting, are rarely hemodynamically significant. Here, we present the case of a neonate with a large unrestrictive Gerbode defect, patent foramen ovale, patent ductus arteriosus, and pulmonary valve insufficiency resulting in a circular intracardiac shunt and cardiogenic shock. The patient was managed with venoarterial extracorporeal membrane oxygenation followed by neonatal Gerbode defect repair.

The mortality rate of postcardiotomy cardiogenic shock after cardiovascular surgery is quite high, and the only way to avoid this serious complication is to initiate a preemptive strategy during surgery. The Impella 5.5 device with the SmartAssist system (Abiomed) is mainly used to prevent or to treat cardiogenic shock in cardiac surgery, but it is not often used in aortic surgery.

A 72-year-old man was diagnosed with posterior ventricular septal rupture after acute myocardial infarction with dissection in the posterior wall of the ventricular septum. On admission, cardiogenic shock was identified, and an Impella CP was inserted to improve his hemodynamics. Extended sandwich patch repair through the right ventricle with a Dacron patch was successfully performed, achieving an excellent postoperative course with stable hemodynamics.
